Job Overview

A Patient Care Navigator - Clinical plays a vital role in assisting patients with their healthcare journey, ensuring they receive the necessary support for managing chronic conditions and scheduling healthcare services. This position is essential in guiding patients through the complexities of the healthcare system, thereby enhancing care continuity and improving overall health outcomes.

Key Responsibilities:
  • Deliver educational resources and information pertaining to the patient's diagnosis and treatment strategies.
  • Guide patients and their families through understanding disease processes, setting achievable health goals, and managing wellness, including transitions of care and chronic care management.
  • Proactively identify potential health crises and implement measures to mitigate risks.
  • Assist patients in accessing healthcare services, social support, and community resources.
  • Engage with clients, families, and caregivers to align with care plan objectives.
  • Organize follow-up care post-hospitalization or emergency visits.
  • Collaborate effectively with healthcare team members and providers to promote optimal health results.
  • Conduct outreach to patients to ensure adherence to treatment protocols.
  • Maintain accurate records and databases related to care coordination efforts.
Qualifications:
  • Current Licensed Practical Nurse (LPN) certification is mandatory.
  • A minimum of 2 years of experience in a healthcare environment, with a preference for primary care exposure.
Essential Skills and Knowledge:
  • Experience in medically-oriented administrative roles.
  • Familiarity with EMR/EHR systems.
  • Proficiency in Microsoft Office applications.
  • Strong collaborative and teamwork abilities.
  • Understanding of medical terminology.
  • Exceptional customer service capabilities.
  • Ability to work autonomously and demonstrate initiative.
  • Comfortable engaging with patients and providing medical education.
  • Knowledge of healthcare and managed care frameworks.
  • Outstanding communication and organizational skills.
