Key Responsibilities:

  • Leverage data analytics to identify and address care gaps in clinical quality metrics.
  • Employ a patient-centered methodology and motivational interviewing techniques to enhance health outcomes.

  • Manage population data effectively using various computer applications.
  • Guide patients and families in accessing, navigating, and connecting with internal services and external community resources. Provide education and outreach as necessary to enhance patients' quality of life and well-being.
  • Support outreach initiatives and yearly plans to link current and prospective patients to services within the health center and other specialty providers.
  • Maintain and report on outreach registries to bolster the outreach framework and strategy.
  • Accurately schedule appointments in accordance with Neighborhood policies.
  • Track and report metrics and outcomes related to outreach efforts.
  • Demonstrate independent problem-solving skills to assist patients and providers in achieving care plan objectives.
  • Advocate for patients when engaging with community agencies and services.
  • Consult with healthcare providers regarding patient needs and facilitate appropriate referrals.
  • Assist patients in obtaining and completing referrals as directed by providers, including securing prior authorizations when necessary. Document and monitor referrals until they are completed.
  • Help patients access health insurance by connecting them with suitable facilitated enrollers.
  • Collaborate as a team member with nursing, providers, community health workers, billing, and other Neighborhood staff to ensure comprehensive patient care.
  • Participate in training sessions and required meetings to promote professional growth and development through educational programs.
  • Contribute to the development of procedures and policies related to job responsibilities.
  • Assist in creating and implementing metrics to evaluate the effectiveness of this role.
  • Ensure thorough and accurate documentation of projects and assignments.
  • Perform other duties as assigned.
Compensation: $19.00 per hour (based on a full-time, 40-hour work week). Individual compensation is determined by various factors unique to each candidate, including skill set, experience, qualifications, and other job-related components.

Qualifications:
  • High School Diploma or equivalent required; Associate's degree preferred.
  • Experience in Community Health, Patient Resource Coordination, or a related field.
  • Proficient computer skills, clerical abilities, data entry, and organizational skills are essential.
  • Excellent written and verbal communication skills are required.
  • Must be able to read, write, and speak English; proficiency in Spanish is preferred.
