Job Overview

Position Summary:

The Patient Admissions Coordinator will deliver daily onsite assistance at designated healthcare facilities for patient assessment and evaluation. This role is pivotal in managing and guiding the admissions process within various healthcare environments, including hospitals, clinics, nursing homes, and similar organizations, under the supervision of the lead admissions coordinator. Additionally, the Patient Admissions Coordinator will participate in on-call rotations to support admissions during evenings and weekends across our network.

Key Responsibilities (including but not limited to):

  • Conduct thorough screenings and evaluations of patients for long-term or subacute care.
  • Submit relevant admissions documentation for review by the lead admissions coordinator.
  • Ensure timely and efficient completion of patient assessments.
  • Manage inquiry calls from community practices regarding screening evaluations and maintain the referral system as directed.
  • Review daily bed availability and facilitate the filling of openings in facilities.
  • Establish and nurture relationships with healthcare staff.
  • Travel between facilities as required for mobile operations.

Qualifications:

  • Must possess a degree in social work, nursing (LPN), or a related health field.
  • At least 1-2 years of experience in the healthcare sector is required.
  • If lacking a degree, a minimum of 5 years of admissions screening experience in a skilled nursing facility is necessary.
  • Experience in admissions screening and long-term care settings is preferred.
  • A valid driver's license and insurance are mandatory.
