Position Overview

ROLE SUMMARY:

This position is part of a supportive housing initiative designed for individuals residing in permanent scattered-site accommodations. Under the general oversight of the Program Director, the Community Support Specialist acts as a role model, providing education on self-help strategies, imparting effective coping mechanisms derived from personal experiences, and teaching skills for symptom management. This role also involves assisting individuals in defining rehabilitation and recovery objectives and fostering the establishment of community support systems and networks.

The Community Support Specialist aids assigned individuals in cultivating and sustaining viable living, working, and social environments by securing necessary medical, social, psychosocial, educational, financial, vocational, and other essential services. This role includes participation in crisis intervention services for participants. Most responsibilities are carried out independently with general supervision from the Program Director. The Community Support Specialist supports individuals through empathetic counseling, encouraging them to engage fully in community life with the aim of achieving personal goals that promote growth and independence.

KEY RESPONSIBILITIES:

  1. Provide counseling to individuals regarding their health and mental health needs, helping them mobilize their inner strengths and available resources to maintain independence within the community.
  2. Educate participants on self-help techniques as part of their social and community integration.
  3. Impart effective coping strategies based on personal and lived experiences.
  4. Instruct on symptom and medication management.
  5. Assist in defining rehabilitation and recovery goals.
  6. Maintain accurate and up-to-date case records for each individual.
  7. Gather and develop resource information for residents regarding all aspects of their care, presenting this information clearly and understandably.
  8. Support individuals in achieving person-centered service plan goals through a Whole Health approach.
  9. Facilitate personalized services that empower individuals to thrive within the community.
  10. Refer individuals to internal services, community resources, and other organizations, accompanying them to medical appointments, treatment facilities, and other relevant locations.
  11. Advocate for individuals with external service providers and within the organization.
  12. Assist individuals in locating and accessing vocational training or other educational opportunities, collaborating with team members to address individual needs.
  13. Provide support for health and mental health, as well as social and community integration.
  14. Adhere to attendance and timekeeping policies, consistently reporting to work as scheduled.
  15. Participate in regular clinical meetings, staff meetings, supervision, and in-service training and development activities.
  16. Follow agency infection control protocols.
  17. Perform other job-related duties as assigned.

ESSENTIAL QUALIFICATIONS:

  1. Ability to engage with residents, families, and staff in a compassionate and respectful manner, with an understanding of cultural differences.
  2. Capacity to serve as a role model for individuals.
  3. Proficiency in completing written forms and reports accurately and timely.
  4. Effective communication skills with staff, residents, families, and the public.
  5. Ability to prepare precise documentation, reports, and other written materials as required.
  6. Skill in securing cooperation and working effectively with others.
  7. Ability to work independently while adhering to safety and accountability measures.
  8. Understanding of how disabilities can impact functioning and strategies to cope with or overcome these challenges.
  9. Proficient reading and writing skills at a minimum of a 12th-grade level, with the ability to follow written and oral instructions.
  10. Capability to accompany or transport residents throughout the city, with some assignments potentially requiring a valid driver's license.
  11. Empathy and supportiveness towards residents, instructing, demonstrating, modeling, and encouraging appropriate behaviors and skills.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Completion of an internship or equivalent training in peer advocacy is preferred; experience in providing advocacy services to individuals with mental health challenges and/or homelessness is desirable.
  • Current or past recipient of mental health, substance use, or homeless services is preferred.
