Position Type: On Call

Role Summary:

The Community Support Specialist plays a vital role in delivering Home and Community Based Services (HCBS) and Children and Family Treatment and Support Services (CFTSS). These services aim to address behavioral health needs early in a child's life, thereby preventing the escalation of mental health issues and reducing the necessity for more intensive interventions.

The Specialist is tasked with offering Community Psychiatric Support (CPST) and Psychosocial Rehabilitation (PSR) to eligible children and their families. Additionally, the role includes providing Caregiver Family Supports, Prevocational Services, Supportive Employment Services, and Community Self-Advocacy Training and Support. Services are primarily delivered in home and community environments.

Moreover, the Specialist will facilitate CPST and PSR during family visitations, employing effective Visit Coaching techniques to enhance service delivery during these critical family interactions.

Key Responsibilities:

  • Engage in the creation and continuous evaluation of the child's Treatment Plan.
  • Deliver direct services to assigned children and families.
  • Provide services across the five boroughs.
  • Maintain thorough documentation of all service interactions and provisions.
  • Participate in all necessary meetings.
  • Coordinate care with other service providers to support the child's optimal functioning within the community.
  • This position is currently remote, with potential reporting to the Brooklyn office as needed.
  • Complete all required training sessions.

Qualifications:

  • Bachelor's degree in social work, psychology, or a related field, along with four years of relevant experience.
  • Master's degree in social work, psychology, or a related field is preferred, with at least one year of relevant experience.
  • Strong writing, communication, and computer skills are essential.
  • Willingness to travel to various communities.
  • Flexibility to work evenings and weekends as required.
  • A valid NYS driver's license is mandatory.

This is an hourly position, with compensation based on experience.
