Reporting To: Assistant Program Director

Program Description: The Cortelyou Supportive Housing initiative is dedicated to providing a safe and nurturing environment for young adults aged 18-25 who are experiencing serious mental health challenges and have a history of homelessness. This program is particularly focused on supporting members of the LGBTQIA+ community, offering comprehensive services aimed at fostering long-term stability.

Key Responsibilities:

  • Foster a secure and inclusive living atmosphere for young adults transitioning from homelessness, particularly those with significant mental health issues.
  • Deliver high-quality services to all clients, adhering to regulatory standards, best practices, and organizational policies.
  • Assist residents in developing essential daily living skills, including budgeting, meal preparation, and personal hygiene.
  • Conduct weekly group sessions to support residents in adapting to independent living, covering topics such as medication management and employment readiness.
  • Collaborate with case managers to help clients achieve their individualized service goals.
  • Facilitate individual sessions focused on skill development and document progress promptly.
  • Provide case management support during program absences to ensure continuity of care.
  • Accompany clients to various appointments as necessary.
  • Connect residents with community resources to enhance their overall well-being.
  • Encourage participation in community activities to promote socialization.
  • Work with the social services team to assist residents with necessary documentation for housing recertification.
  • Participate in safety skills training and assist with resident transitions.
  • Attend required meetings, trainings, and workshops.
  • Support community initiatives and programs.
  • Perform additional tasks as assigned.

Qualifications:

  • Bachelor's degree in a human services-related field is required; a Master's degree is preferred.
  • Experience working with homeless or at-risk LGBTQIA+ youth is highly desirable.
  • Strong communication skills, both written and verbal.
  • Excellent organizational and time-management abilities.
  • A commitment to learning and adapting to new challenges.
  • Ability to engage with young people and foster supportive relationships.
  • Proficient in collaborative work with families and external partners.
  • Reliable and consistent in professional conduct.
  • Promote diversity and believe in the potential for change.
  • Maintain appropriate professional boundaries and manage stress effectively.

Technical Skills:

  • Ability to navigate challenges and advocate for clients.
  • Willingness to learn data interpretation for informed decision-making.
  • Experience in team collaboration and relationship building.
  • Knowledge of community resources and services.
  • Background checks and clearances are required for this position.
