POSITION SUMMARY:
This role is situated within a supported housing initiative aimed at assisting individuals residing in permanent scattered-site accommodations. Under the guidance of the Program Director, the Community Support Specialist acts as a mentor, providing education on self-help strategies, imparting effective coping mechanisms derived from personal experiences, and facilitating the development of symptom management skills. The Specialist aids in defining rehabilitation and recovery objectives while fostering community support systems and networks.
The Community Support Specialist plays a crucial role in helping assigned individuals cultivate and sustain meaningful living, working, and social environments by accessing essential medical, social, psychosocial, educational, financial, vocational, and other necessary services. This position also involves providing crisis intervention services to participants. Most responsibilities are carried out independently, with general oversight from the Program Director. The Specialist supports individuals through empathetic counseling, encouraging full participation in community life to achieve personal goals that promote growth and independence.
KEY RESPONSIBILITIES:
- Provide counseling to individuals regarding their health and mental health needs, empowering them to utilize internal strengths and community resources to maintain independence.
- Educate clients on self-help techniques to enhance social and community integration.
- Instruct residents on effective coping strategies based on personal and lived experiences.
- Guide individuals in symptom and medication management.
- Assist in clarifying rehabilitation and recovery objectives.
- Maintain accurate and up-to-date case records for each individual.
- Gather and disseminate resource information relevant to residents' care in a clear and comprehensible manner.
- Support individuals in achieving service plan goals that are centered around their personal needs and a holistic health approach.
- Facilitate tailored services that empower individuals to thrive within the community.
- Refer individuals to internal services, community resources, and external organizations, accompanying them to medical appointments and other necessary locations.
- Advocate for individuals with external service providers and within the organization.
- Assist individuals in locating vocational training or other educational opportunities, collaborating with team members to address individual needs.
- Provide support for health and mental health, fostering social and community integration.
- Adhere to attendance and timekeeping protocols, reporting to work reliably and consistently.
- Participate in scheduled clinical meetings, staff meetings, supervision, and professional development activities.
- Comply with agency infection control policies.
- Perform other job-related duties as assigned.
ESSENTIAL QUALIFICATIONS:
- Ability to engage with residents, families, and staff in a compassionate and respectful manner, recognizing and valuing cultural differences.

- Proficiency in completing written documentation and reports accurately and promptly.
- Effective communication skills with staff, residents, families, and the public.
- Ability to prepare timely and accurate documentation, reports, and other written materials as required.

- Capacity to work independently while adhering to safety and accountability standards.
- Understanding of how disabilities can impact functioning and strategies to cope with or overcome these challenges.
- Ability to read and write at a minimum of a 12th-grade level and follow both written and oral instructions.
- Willingness to accompany or transport residents throughout the city as needed (some roles may require a valid driver's license).
- Empathy and supportiveness towards residents, demonstrating and encouraging appropriate behaviors and skills.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Completion of an internship or similar training in peer advocacy is preferred; experience in providing advocacy services to individuals with mental health issues and/or homelessness is advantageous.
- Current or past experience as a recipient of mental health, substance use, or homelessness services is preferred.
