JOB ANNOUNCEMENT


Sacred Heart Community Service is seeking a dedicated Housing Stability Case Specialist to assist families at risk of losing their homes. This role involves providing financial support, temporary case management, and facilitating connections to essential community resources.

The ideal candidate will be a proactive individual with exceptional organizational skills and attention to detail. Strong communication abilities and experience in case management are essential, along with a deep understanding of the challenges faced by low-income families. A passion for our mission to alleviate poverty and improve lives is crucial.

For over five decades, SHCS has been a leading force in combating poverty in the region, employing a dual strategy that addresses immediate needs while advocating for systemic change.


POSITION SUMMARY


The Housing Stability Case Specialist will work closely with low-income families to prevent or resolve homelessness by offering one-time and short-term financial aid, strength-based case management, advocacy, landlord mediation, outreach, and other supportive services aimed at achieving stable, permanent housing.

This role involves screening, assessing, and conducting intakes for potential participants. The Specialist will provide enrolled families with adaptable financial assistance, temporary case management, and other support, employing a creative approach to ensure families maintain permanent housing.

This position is part of the Family Assistance Team and reports to the Family Assistance Manager.


RESPONSIBILITIES AND DUTIES

Case Management (75-85%)

  • Conduct prescreens and intakes to assess participant eligibility and determine the necessary level of assistance.
  • Engage families through comprehensive assessments of strengths and needs, developing goals and service plans to ensure housing stability.
  • Manage a flexible caseload of up to 30 households, including those receiving one-time financial aid and those in ongoing case management.
  • Provide information, referrals, and connections to community resources to support stable housing.
  • Advocate for families in maintaining stable housing, including educating them on tenant rights and facilitating landlord mediation.
  • Assist families in locating and securing new housing when necessary.
  • Support families with financial literacy and budget counseling.
  • Provide immediate crisis intervention and problem-solving assistance as needed.
  • Connect families to additional SHCS resources, including essential services and self-sufficiency programs.
  • Maintain regular communication with households post-assistance to assess housing status and offer further support.
  • Document all program activities, including prescreens, intakes, and case management in designated databases within specified timeframes.
  • Maintain case files and progress notes, adhering to professional standards.
  • Implement program evaluation tools and report on outcomes.
  • Submit required monthly reports and process financial assistance payments as needed.

Participation and Leadership (10-20%)

  • Engage and develop program volunteers through education and support.
  • Assist in the recruitment and development of leaders through training and committee support.
  • Collaborate with the Family Assistance Team to ensure comprehensive coverage of responsibilities.
  • Participate in agency-wide programs and outreach efforts.
  • Attend staff meetings and contribute to team events.
  • Perform other assigned duties as necessary.

Outreach and Engagement (5%)

  • Promote the availability of homelessness prevention assistance through community engagement.
  • Develop collaborative relationships with community partners, including schools and local organizations.

REQUIRED QUALIFICATIONS AND EXPERIENCE:

  • Strong commitment to the vision of a community free from poverty.
  • Bachelor's degree in social work, public health, or a related field, or equivalent experience.
  • Three years of experience in human services or a related field.
  • Bilingual and bicultural in English-Spanish, both written and verbal.
  • Experience in culturally competent, strength-based case management.
  • Knowledge of intake, assessment, service planning, and crisis intervention processes.
  • Excellent communication skills, with the ability to convey difficult messages compassionately.
  • Sensitivity to issues faced by households experiencing homelessness or other challenges.
  • Ability to thrive in a dynamic environment while maintaining a positive approach.
  • Collaborative team player with the ability to work independently.
  • Proficient in MS Office Suite and ability to learn new database systems.
  • Valid driver's license and reliable vehicle.
  • Background check required.

PREFERRED QUALIFICATIONS AND EXPERIENCE:

  • Two years of experience in outreach or supportive housing programs.
  • Experience working with landlords and knowledge of local rental markets.

REPORTING RELATIONSHIP:
The Housing Stability Case Specialist reports to the Family Assistance Manager.

This is a non-exempt, hourly position with a competitive starting rate.
