Position Overview:

Benchmark Human Services is a leading organization dedicated to supporting individuals with intellectual and developmental disabilities (IDD) and behavioral health needs. Our mission is to empower individuals to achieve their fullest potential through various services including residential support, employment assistance, crisis intervention, and community engagement. We are currently seeking a Supported Living Manager to oversee our Supported Living homes in South Bend, IN.

Key Responsibilities:

  • Oversee the daily operations of Supported Living homes, ensuring compliance with state and federal regulations.
  • Supervise and mentor staff, fostering a supportive and productive work environment.
  • Collaborate with a multidisciplinary team to develop and implement Individual Support Plans (ISP) and Person Centered Plans (PCP) for each client.
  • Engage in direct service delivery to clients as needed.
  • Maintain accurate documentation in accordance with organizational and regulatory standards.
  • Manage client budgets and assist in maintaining Medicaid eligibility through proper documentation and scheduling.
  • Communicate effectively with families, case managers, healthcare professionals, and other stakeholders.
  • Participate in various meetings, including team meetings and client planning sessions.
  • Implement training programs tailored to each client's ISP.
  • Ensure adherence to behavior support policies and procedures.
  • Assist in the recruitment and performance management of staff.
  • Coordinate medical appointments and ensure clients receive necessary healthcare services.
  • Plan and facilitate community-based recreational activities for clients.
  • Respond to on-call situations with professionalism and urgency.
  • Report any concerns regarding abuse, neglect, or exploitation to appropriate authorities.
  • Maintain required certifications and training as mandated by federal, state, and organizational standards.
  • Be physically capable of lifting up to 50 pounds and maintaining mobility throughout the workday.

Qualifications:

  • Bachelor's degree is preferred.
  • Minimum of two years of experience working with individuals with disabilities.
  • At least one year of supervisory experience is preferred.
  • Current CPR and First Aid certifications.
  • Valid driver's license and auto insurance are required.
  • Completion of necessary training in areas such as interviewing, hiring, and employee relations.
