Job Overview

The Senior Care Support Specialist plays a vital role in enhancing the quality of life for families caring for individuals with Alzheimer's or age-related dementia. This position requires a dedicated professional who possesses the necessary experience and personal attributes to ensure that services and activities are tailored to meet the unique needs of each participant.

Key Responsibilities:

  1. Ensure that program activities align with the individual and collective needs of participants and their families, providing direct care and services as necessary, including medication assistance.
  2. Collaborate with the Adult Day Center Manager to design, schedule, and implement engaging recreational and daily living activities.
  3. Maintain accurate records and prepare reports detailing monthly activities, including participant case notes as required.
  4. Exhibit essential professional qualities to guarantee that services and activities are delivered effectively and in accordance with participant requirements.
  5. Attend to the basic personal needs of participants, identify critical health conditions, and coordinate necessary care actions while supporting fellow staff members.
  6. Respond to emergency care requests as needed.
  7. Assist in the development of individualized care plans for participants, regularly reviewing and assessing their conditions, and communicating any changes to case managers.
  8. Support staff in planning and preparing monthly snack menus, including the preparation and cleanup of meals.
  9. Contribute to maintaining a clean and orderly environment, ensuring that necessary supplies and safety measures are in place.
  10. Ensure the security of the facility by following proper opening and closing procedures.
  11. Adhere to all agency policies and procedures.
  12. Input participant data and case notes into the electronic medical record system, complying with agency protocols and regulatory requirements.
  13. Participate in training sessions, in-service meetings, and staff gatherings.
  14. Maintain compliance with training requirements as outlined by agency policies and relevant regulations.
  15. Provide additional support services and perform other duties as assigned.

Qualifications:

  1. High School diploma or equivalent.
  2. Two years of relevant experience in human or social services, particularly with seniors experiencing Alzheimer's or related dementia.
  3. Preferred: Six months of experience working with individuals with developmental disabilities.
  4. Must meet agency insurance criteria for operating vehicles for agency business.

Skills and Abilities:

Strong communication skills with participants, families, and colleagues. Ability to assess needs and coordinate both individual and group activities. Proficiency in direct care practices and training. Reliable transportation and a clean driving record are essential. Familiarity with computer systems and electronic medical records is required, along with the ability to learn new software applications.

Licenses and Certifications:

Valid Alaska Driver's License.

  • Adult CPR Certification
  • First Aid Certification
  • Geri-Care Certification

Immunization Requirements:

Annual PPD Test and Hepatitis B immunization (or waiver).

Working Conditions:

This role involves medium physical activity, including the ability to exert up to 50 pounds of force occasionally and up to 20 pounds frequently.

Physical Activities:

Includes stooping, crouching, reaching, standing, walking, pushing, pulling, lifting, talking, hearing, and driving as required by state law.

Environmental Conditions:

The position is primarily office-based with minimal exposure to adverse environmental conditions.
