Community Support Specialist

POSITION SUMMARY:
The Community Support Specialist is responsible for executing a tailored habilitation strategy for eligible individuals while delivering consistent direct assistance in various settings, including home, community, or facility-based environments. This role collaborates closely with families or guardians and the Home/Community Services Manager to create, implement, and adjust effective intervention methods that can be seamlessly integrated into the daily lives of families or guardians.
KEY RESPONSIBILITIES:
1. Provides direct support according to the established Plan of Care (POC), which may encompass aiding individuals in acquiring, maintaining, and enhancing self-care, social, and adaptive skills such as meal preparation, personal hygiene, and leisure activities.
2. Ensures that the health and safety needs of participants are consistently met.
3. Prepares meals or assists individuals in meal preparation, ensuring nutritional requirements are fulfilled during care.
4. Monitors, supervises, and documents participant activities, discussing observations with the Home/Community Manager and other staff to refine intervention strategies as needed.
5. Maintains accurate records and generates reports regarding the services provided.
6. Responds to emergencies and provides assistance as necessary.
7. Keeps abreast of current knowledge related to developmental disabilities.
8. Inputs participant data and case notes into the electronic medical record system, adhering to agency protocols and applicable regulations.
9. Participates in training sessions, in-service workshops, and staff meetings as required.
10. Ensures compliance with training requirements in line with agency policies and relevant regulations.
11. Performs additional support services and responsibilities as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:
1. High School Diploma or equivalent is required.
2. Preferred but not mandatory: Six months of experience working with individuals with developmental disabilities.
3. Must meet the agency's insurance criteria for operating vehicles for agency-related activities.
SKILLS AND QUALIFICATIONS:
Demonstrated ability in direct care tasks. Strong communication skills, both verbal and written, to interact effectively with participants, families, supervisors, and colleagues. Excellent observational and organizational abilities to assess and document participant responses to interventions. Flexibility in scheduling and the capacity to work independently while prioritizing tasks with minimal supervision. Reliability, professionalism, and enthusiasm are essential. Must possess reliable transportation and maintain a clean driving record. Proficiency in using computers and electronic medical record systems is required.
LICENSES AND CERTIFICATIONS:
Valid Alaska Driver's License.
Agency Provided Certifications:
- Certification in Adult/Child and Infant CPR.
- Certification in First Aid.
- Safety-Care Certification.
IMMUNIZATION REQUIREMENTS:
Annual PPD Test.
Hepatitis B immunization (or a waiver).
WORKING ENVIRONMENT:
Medium Work: Involves exerting up to 50 pounds of force occasionally, up to 20 pounds frequently, and up to 10 pounds constantly to move objects.
PHYSICAL DEMANDS:
Includes activities such as stooping, crouching, reaching, standing, walking, pushing, pulling, and lifting.
ENVIRONMENTAL CONDITIONS:
Generally, the worker is not exposed to adverse environmental conditions.
