Job Overview

The Senior Care Support Specialist plays a vital role in enhancing the quality of life for families caring for individuals with Alzheimer's or age-related dementia. This position requires a compassionate individual with the necessary experience and personal qualities to ensure that activities and services are tailored to meet the unique needs of each participant.

Key Responsibilities:

  1. Ensure that program activities align with the individual and collective needs of participants and their families, providing direct care and assistance with medication as necessary.
  2. Collaborate with the Adult Day Center Manager to plan, schedule, and implement recreational, daily living, and leisure activities.
  3. Maintain accurate records and prepare reports on monthly activities, including daily attendance and case notes for participants.
  4. Exhibit essential experience and personal traits to guarantee that activities and services are delivered in a professional and appropriate manner, in accordance with participant needs.
  5. Attend to participants' basic personal needs, identify critical health or physical conditions, and determine necessary care actions while supporting staff and notifying case managers as needed.
  6. Provide or request emergency care as required.
  7. Assist in the development of participant care plans, regularly reviewing and assessing participant conditions, and communicating any changes to appropriate case managers.
  8. Support staff in planning monthly snack menus, preparing snacks, and ensuring cleanliness after meals.
  9. Help maintain a clean and orderly environment, ensuring that necessary snacks, supplies, and equipment are available while observing safety protocols.
  10. Ensure the security of the senior services facility by following opening and closing procedures.
  11. Implement all agency policies as required.
  12. Input participant information and case notes into the electronic medical record system, adhering to agency procedures and regulatory requirements.
  13. Participate in training seminars, in-service sessions, and staff meetings.
  14. Maintain compliance with training requirements as per agency policies and state and federal regulations.
  15. Provide additional support services and perform other duties as necessary.

Minimum Qualifications:

  1. High School diploma or equivalent.
  2. Two years of relevant experience in human or social services, particularly with seniors experiencing Alzheimer's or related dementia (preferred).
  3. Six months of experience working with individuals with developmental disabilities is preferred but not mandatory.
  4. Must meet agency insurance criteria for operating agency and personal vehicles for agency-related tasks.

Knowledge, Skills, and Abilities:

Demonstrated ability to communicate effectively with participants, families, and colleagues. Proven capability to assess needs and coordinate both individual and group activities. Proficiency in direct care activities and training is essential. Reliable transportation and a clean driving record are required. Familiarity with computer systems and electronic medical records (EMR) is necessary, along with the ability to learn new software and hardware applications.

Licenses/Certifications:

Valid Alaska Driver's License.

Agency Provided Certifications:

  • Certification in Adult CPR.
  • Certification in First Aid.
  • Geri-Care Certification.

Immunization Requirements:

Annual PPD Test.

Hepatitis B immunization (or waiver).

Working Conditions:

Medium Work: Exerting up to 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects.

Physical Activities:

Includes stooping, crouching, reaching, standing, walking, pushing, pulling, lifting, talking, hearing, and driving as required by the job.

Environmental Conditions:

None: The worker is not substantially exposed to adverse environmental conditions typical of office or administrative work.
