Position Summary: The Patient Care Coordination Specialist plays a vital role in the healthcare team, serving as an advocate for patients with chronic illnesses or those at high risk. This position involves close collaboration with patients, their families, healthcare providers, and community resources to ensure a seamless continuum of care.

Key Responsibilities:

  • Independently manage and track patient care, ensuring effective discharge planning and educational support for patients and their families.
  • Contribute to the development of individualized care plans tailored to the specific needs of patients.
  • Work alongside a multidisciplinary team to implement clinical pathways that enhance patient care delivery.
  • Coordinate care across various healthcare settings, ensuring a smooth transition from hospitalization to home or further care.
  • Monitor quality measures related to care coordination and identify opportunities for improvement.
  • Conduct thorough reviews of patient records to identify adverse events and areas for enhanced care.
  • Analyze clinical data to assess treatment efficacy and recommend necessary adjustments to care protocols.
  • Facilitate physician peer review committees and present findings to relevant stakeholders.
  • Collaborate with medical staff to evaluate the quality and appropriateness of patient care.
  • Engage in hospital-wide quality improvement initiatives and maintain compliance with healthcare standards.
  • Participate in community outreach and educational programs to enhance professional knowledge and share insights with peers.

Qualifications:

  • 3 to 5 years of relevant experience in healthcare coordination or case management.
  • Experience with Utilization Review and familiarity with clinical guidelines.
  • Bachelor's degree in Nursing or a related field is required; a Master's degree is preferred.

Skills:

  • Strong analytical and organizational abilities, with a focus on prioritizing tasks effectively.
  • Excellent communication skills, both verbal and written, with the ability to engage with diverse stakeholders.
  • Proficient in using healthcare technology and software applications.
  • Ability to maintain professionalism in high-pressure situations and exercise sound judgment.

Credentials:

  • Valid RN license in Florida is required.
  • Basic Life Support (BLS) certification and any additional relevant certifications are necessary.
