Our multidisciplinary care team includes Physicians, Advanced Practice Providers, Pharmacists, Care Coach Nurses, Medical Assistants, Behavioral Health Specialists, Quality Coders, Referral Coordinators, and more. This integrated model is designed to enhance the patient experience, particularly for seniors, by offering a comprehensive suite of services, coordinated care, and strong community ties.
Key Responsibilities:- Assess and manage patient care in alignment with established medical standards.
- Monitor and evaluate the quality of care using available data and chart reviews.
- Facilitate the coordination of patient services, including specialty referrals and home health care.
- Engage actively with the care team during daily meetings to share medical insights.
- Collaborate with leadership to foster a cooperative practice environment.
- Complete medical documentation promptly, working alongside quality coders to enhance coding accuracy.
- Participate in discussions regarding care quality, outcome reviews, and policy adherence.
- Contribute to the growth of new or existing services within the organization.
- Engage in the local primary care on-call program as required.
- Ensure compliance with all relevant licensing and accreditation standards.
- Dedicate 100% of your time to direct patient care, including administrative tasks related to patient interactions.
• Valid and unrestricted medical license or willingness to obtain one in the relevant state.
• Graduate from an accredited MD or DO program.
• Strong verbal and written communication abilities.
• Exceptional interpersonal skills with colleagues and patients.
• Commitment to the integrated team-based care model.
• Adaptability to a value-based care environment.
• Excellent patient service skills.
• Basic computer proficiency, including email and electronic medical records.
• Participation in TB screening program is mandatory for this patient-facing role.
• Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
• Active DEA license.
• Medicare and Medicaid Provider Numbers.
• Two to five years of relevant experience is preferred.
• Experience managing a Medicare Advantage patient panel with a focus on coordinated care best practices.
• Familiarity with Medicare guidelines and quality indicators.
