Become an integral part of our dedicated team and prioritize health. The Primary Care Physician (PCP) serves as a pivotal leader within our collaborative care framework.
As a value-oriented healthcare provider, we emphasize quality service for our patients. Our multidisciplinary care team includes Physicians, Advanced Practice Providers, Pharmacists, Care Coach Nurses, Medical Assistants, Behavioral Health Specialists, Quality Coders, Referral Coordinators, and more.
This comprehensive support structure enables our PCPs to manage a reduced patient load, allowing for more meaningful interactions with each individual.
Key Responsibilities:
- Assess and treat patients in alignment with established care standards.
- Monitor patient care quality through data analysis and chart evaluations.
- Facilitate coordination of patient services, including specialty referrals and home health care.
- Engage actively with the care team during daily meetings, contributing medical expertise.
- Collaborate with leadership to foster a cooperative practice environment.
- Ensure timely completion of medical documentation, optimizing coding accuracy with quality coders.
- Adhere to clinical policies and protocols.
- Discuss care quality and outcomes with leadership regularly.
- Explore opportunities for service expansion within the center.
- Participate in the local primary care on-call program as necessary.
- Maintain compliance with all licensing and accreditation requirements.
- Dedicate 100% of your time to direct patient care, including administrative tasks related to patient interactions.
Required Qualifications:
- Current and unrestricted medical license or willingness to obtain one in the practice state.
- Graduate from an accredited MD or DO program.
- Strong verbal and written communication abilities.
- Proven interpersonal skills with colleagues and patients.
- Commitment to the integrated team-based care model.
- Adaptability to a value-based care environment.
- Excellent patient service skills.
- Basic computer proficiency, including email and electronic medical records.
Preferred Qualifications:
- Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Active DEA license.
- Medicare and Medicaid Provider Numbers.
- Two to five years of relevant experience preferred.
- Familiarity with managing Medicare Advantage patients and knowledge of Medicare guidelines.
