Position Summary:
We are currently seeking a Senior Practical Nurse Specialist to join our dedicated team. This role is essential in coordinating patient care and ensuring effective communication between patients and healthcare providers.

Key Responsibilities:
1. **Patient Coordination:**
- Manage patient referrals to specialty physicians and outpatient services.
- Facilitate interdisciplinary team referrals and track clinical referrals for care transition.

2. **Financial Assistance Coordination:**
- Lead the coordination of financial assistance referrals, collaborating with various stakeholders to support patients in obtaining necessary approvals.

3. **Patient Education:**
- Provide comprehensive education and instructions to patients and families as directed by healthcare providers, ensuring all interactions are documented accurately.

4. **Clinical Review:**
- Review laboratory and diagnostic results, promptly communicating critical findings to the physician.

5. **Quality Improvement Participation:**
- Engage in quality improvement initiatives, evaluating patient care quality and satisfaction, and recommending enhancements to meet patient needs.

6. **Mobile Health Support:**
- Assist with mobile health services, including vital signs, intake, and documentation of care provided in electronic medical records.

Qualifications:
- High school diploma or equivalent with three years of relevant nursing experience, or an equivalent combination of education and experience.
- Licensed Practical Nurse certification and Basic Cardiac Life Support certification required.
- Preferred: Graduate of an accredited Practical Nursing program, with at least two years of experience in a clinical setting, along with phlebotomy and immunization administration skills.
