The University of Miami Health System is excited to announce a full-time opportunity for a Patient Navigator. This role is essential in facilitating patients' access to necessary services by identifying obstacles to care, coordinating treatment plans, and overseeing the execution and follow-up of services prescribed by healthcare providers.

The Patient Navigator plays a crucial role in enhancing patient experiences by:

  1. Conducting thorough patient assessments to gather essential information for effective treatment planning.
  2. Ensuring patients are informed about insurance and social service programs available to them.
  3. Providing reminders for upcoming appointments and serving as a liaison between patients and healthcare providers.
  4. Identifying and addressing any barriers to care, whether real or perceived.
  5. Educating and counseling patients on health-related issues and personal circumstances.
  6. Staying informed about community health resources and programs available to patients.
  7. Adhering to University policies and safeguarding institutional assets.

This list of responsibilities is not exhaustive and may expand to include additional duties as necessary.


CORE QUALIFICATIONS
Education:
High School diploma or equivalent required.
Experience:
Minimum of 3 years of relevant experience required, preferably in a medical office or hospital setting.

Knowledge, Skills, and Attitudes:
Ability to manage confidential information with discretion. Proficient communication skills in both oral and written forms. Capacity to handle challenging and stressful situations with professionalism. Ability to maintain effective interpersonal relationships and follow instructions.

Department Specific Functions:
Maintains the Welcome/Patient Resource Center. Provides patient education regarding cancer support services. Acts as a liaison to services and departments within the health system.
  • Offers wayfinding services and tours to patients when necessary.
  • Participates in and supports new patient orientation sessions.
  • Maintains confidentiality of patient information and performs follow-up calls for Patient Satisfaction Surveys.
  • Assists with clerical duties such as faxing, scanning, copying, and ordering supplies.
  • Performs other duties as assigned.

Knowledge, Skills, and Abilities:
Excellent customer service skills are essential. Strong written and verbal communication skills are required. Familiarity with medical terminology is preferred. Ability to manage multiple tasks simultaneously.
