The University of Miami Health System is excited to announce a full-time opportunity for a Patient Navigator. This role is essential in enhancing patient access to our comprehensive healthcare services.

The Patient Navigator plays a pivotal role in assisting patients by identifying obstacles to care, coordinating the formulation of treatment strategies, and overseeing the execution and follow-up of services prescribed by healthcare providers.

Key Responsibilities:

  1. Conduct patient assessments to gather essential information for creating effective treatment plans.
  2. Ensure patients are informed about insurance and social service programs for which they qualify.
  3. Provide reminders for upcoming appointments and serve as a liaison between patients and healthcare providers.
  4. Identify and address any real or perceived barriers to accessing care.
  5. Educate and counsel patients on health-related issues and personal circumstances.
  6. Stay updated on community health resources and programs available to patients.
  7. Adhere to University policies and procedures while safeguarding University assets.

This list of responsibilities is not exhaustive and may be expanded to include additional duties as necessary.


Qualifications:

Education: High School diploma or equivalent required.

Experience: Minimum of 3 years of relevant experience required, preferably in a medical office or hospital setting.


Skills and Attributes:

Ability to manage confidential information with discretion. Strong communication skills in both oral and written forms. Capacity to handle challenging and stressful situations with professionalism. Ability to maintain effective interpersonal relationships and follow instructions accurately.

Department Specific Functions:

  • Manage the Welcome/Patient Resource Center.
  • Provide patient education regarding available support services.
  • Assist patients as a liaison to various services and departments.
  • Offer wayfinding assistance and tours to patients when necessary.
  • Participate in new patient orientation sessions.
  • Maintain confidentiality of patient information and conduct follow-up calls for patient satisfaction surveys.
  • Perform clerical duties, including faxing, scanning, copying, and ordering supplies.
  • Complete other assigned tasks as needed.

Essential Skills:

Excellent customer service skills are essential. Strong written and verbal communication skills are required. Familiarity with medical terminology is preferred. Ability to manage multiple tasks simultaneously.
