Key Responsibilities:

  • Offer personalized support to patients, families, and caregivers to navigate healthcare challenges, ensuring culturally sensitive care tailored to the My Wellness Check Initiative.
  • Act as the primary liaison for patients, facilitating communication through both remote and in-person interactions.
  • Foster strong connections with cancer survivors to promote engagement in their medical care and adherence to health assessments.
  • Identify and address barriers to care by connecting patients with essential resources and guiding them through healthcare systems.
  • Collaborate closely with the My Wellness Check and Cancer Survivorship Program Teams to support training and administrative tasks, while establishing professional relationships with clinical staff.
  • Monitor the My Wellness Check patient interface, providing daily updates to the team regarding patient interactions and any technical issues.
  • Assist in the ongoing evaluation of the My Wellness Check program through the development and analysis of patient satisfaction metrics.
  • Prepare and organize patient communication reports, ensuring all interactions are documented accurately.
  • Adapt to evolving work demands and provide clerical support for in-service training sessions.
  • Uphold the highest standards of patient privacy and care compliance.

Qualifications:
Applicants should possess a high school diploma or equivalent, with a minimum of three years of relevant experience, preferably in a medical office or hospital setting. Proficiency in both oral and written communication in English and Spanish is preferred.

Core Competencies:
Successful candidates will demonstrate the ability to build rapport with patients, possess basic oncology knowledge, and maintain compliance with regulatory guidelines. Strong problem-solving skills, attention to detail, and effective communication abilities are essential.
