Overview: The Enhanced Care Management (ECM) Manager at South Central Family Health Center is tasked with overseeing and directing the daily functions of the ECM department, ensuring the provision of exceptional, comprehensive care to patients with intricate healthcare requirements. The Manager will formulate and execute strategic initiatives, uphold compliance with federal and state regulations, and foster a culture of continuous enhancement in patient care. Additionally, the Manager will supervise ECM Case Managers and support their professional growth.
  • Oversee and manage the daily functions of the Enhanced Care Management department, including staffing, scheduling, and coordination of patient care services;
  • Assess and analyze the performance of the ECM department, employing data-driven methods to pinpoint areas for enhancement and implement necessary adjustments;
  • Supervise, mentor, and evaluate the performance of ECM Case Managers, providing guidance and training to ensure optimal patient outcomes and service delivery;
  • Act as the primary liaison for members enrolled in the ECM Program, advocating for them to navigate the healthcare system effectively;
  • Responsible for training ECM Caseworkers and conducting thorough risk assessments and care planning in collaboration with members to develop a Patient-Centered Care Plan;
  • Monitor the execution of Care Plans and recommend modifications as needed to achieve the member's objectives;
  • Ensure adherence to federal, state, and local regulations, as well as SCFHC policies and procedures;
  • Establish and nurture strong relationships with community partners and stakeholders, promoting the services provided by the ECM Department and advocating for the needs of patients with complex healthcare conditions;
  • In collaboration with the Behavioral Health Director, coordinate audits and ensure compliance with all contractual and regulatory requirements;
  • Collaborate with all SCFHC departments (Primary Care, Dental, Behavioral Health, IT, Finance, etc.) to implement ECM program goals and objectives;
  • Ensure ongoing compliance with all ECM criteria at SCFHC.
Qualifications:
  • High School Diploma - Required;
  • Bachelor's Degree in healthcare, behavioral healthcare, or a related field with two years of relevant experience in a Behavioral Health system of care - Preferred;
  • Master's Degree in healthcare, behavioral healthcare, or a related field with one year of relevant experience in a Behavioral Health system of care - Preferred;
  • Experience supervising personnel is preferred;
  • Current California Driver's License and Automobile Insurance.
Travel Requirements:
  • Occasional travel within the region as needed for outreach, project partner coordination, training, etc.
