Overview:
The Director of Enhanced Care Services is responsible for overseeing and managing the daily functions of the Enhanced Care Management (ECM) department, ensuring the provision of high-quality, comprehensive care to patients with complex healthcare needs.

This role involves developing and executing strategic initiatives, ensuring compliance with federal and state regulations, and fostering a culture of continuous improvement in patient care.

The Director will supervise ECM Case Managers, facilitating their professional growth and development.

Key Responsibilities:

  • Manage and oversee the daily operations of the Enhanced Care Management department, including staffing, scheduling, and coordination of patient care services.
  • Monitor and assess the performance of the ECM department, utilizing data-driven methods to identify areas for enhancement and implement necessary changes.
  • Supervise, support, and evaluate the performance of ECM Case Managers, providing guidance, training, and mentorship to ensure optimal patient outcomes and service delivery.
  • Act as the primary contact for members enrolled in the ECM Program, advocating for them to navigate the healthcare system effectively.
  • Conduct comprehensive risk assessments and care planning in collaboration with members to develop a Patient-Centered Care Plan.
  • Monitor the implementation of Care Plans and recommend revisions or updates as necessary to achieve the member's goals.
  • Ensure compliance with federal, state, and local regulations, as well as SCFHC policies and procedures.
  • Establish and maintain strong relationships with community partners and stakeholders, promoting the services offered by the ECM Department and advocating for the needs of patients with complex healthcare conditions.
  • Coordinate audits with the Behavioral Health Director to ensure compliance with all contractual and regulatory mandates.
  • Collaborate with all SCFHC departments (Primary Care, Dental, Behavioral Health, IT, Finance, etc.) to implement ECM program goals and objectives.
  • Ensure ongoing SCFHC compliance with all ECM criteria.

Qualifications:

  • High School Diploma required.
  • Bachelor's Degree in healthcare, behavioral healthcare, or a related field, with two years of experience directly related to core duties within a Behavioral Health system of care preferred.
  • Master's Degree in healthcare, behavioral healthcare, or a related field, with one year of experience directly related to core duties within a Behavioral Health system of care preferred.
  • Experience supervising personnel is preferred.

Additional Requirements:

  • Current California Driver's License and Automobile Insurance.
  • Occasional travel within Los Angeles County and adjacent counties as needed for outreach, project partner coordination, training, etc.
