We are seeking a passionate and dedicated Enhanced Care Management Lead Case Manager to join our team. This role will play a vital part in connecting with our community, identifying individuals who would benefit from our enhanced care management program, and guiding them towards improved health outcomes.
Responsibilities:- Community Engagement: Actively engage with the community to raise awareness about our Enhanced Care Management (ECM) program and identify potential members who could benefit from its services.
- Eligibility Determination & Enrollment: Assess individuals' eligibility for the ECM program and guide them through the enrollment process, ensuring a smooth and supportive experience.
- Care Plan Development & Implementation: Collaborate with members to develop personalized Care Plans that address their unique physical and psychosocial needs, incorporating their goals and preferences.
- Case Management Support: Provide ongoing support and guidance to ECM members, assisting them in navigating healthcare systems, accessing resources, and achieving their health goals.
- Advocacy & Resource Connection: Act as a strong advocate for members, connecting them with essential social services such as housing, transportation, and other community resources that contribute to their overall well-being.
- Collaboration & Communication: Maintain effective communication with clinical providers, case managers, and other stakeholders involved in the member's care, ensuring seamless coordination of services.
- Documentation & Reporting: Accurately document all interactions and progress within the Electronic Health Record (EHR), adhering to established guidelines and protocols.
