Overview:
The Enhanced Care Management (ECM) Director is accountable for overseeing and directing the daily functions of the ECM department, ensuring the provision of high-quality, comprehensive care to patients with intricate healthcare needs.

The Director will strategize and execute initiatives, uphold compliance with federal and state regulations, and foster a culture of continuous enhancement in patient care.

Additionally, the Director will supervise ECM Case Managers, facilitating their professional growth.

Key Responsibilities:

  • Manage and coordinate the daily operations of the Enhanced Care Management department, including staffing, scheduling, and patient care service coordination.
  • Monitor and assess the ECM department's performance, employing data-driven methods to pinpoint areas for enhancement and implement necessary adjustments.
  • Supervise, support, and evaluate ECM Case Managers, providing mentorship and training to ensure optimal patient outcomes and service delivery.
  • Act as the primary liaison for members enrolled in the ECM Program, advocating for their needs to help them navigate the healthcare system.
  • Conduct comprehensive risk assessments and care planning in collaboration with members to create a Patient-Centered Care Plan.
  • Oversee the implementation of Care Plans, recommending updates as needed to achieve members' goals.
  • Ensure adherence to federal, state, and local regulations, as well as SCFHC policies and procedures.
  • Establish and maintain robust relationships with community partners and stakeholders, promoting ECM services and advocating for patients with complex healthcare conditions.
  • Collaborate with the Behavioral Health Director to coordinate audits and ensure compliance with all contractual and regulatory requirements.
  • Work with all SCFHC departments (Primary Care, Dental, Behavioral Health, IT, Finance, etc.) to achieve ECM program objectives.

Qualifications:

  • High School Diploma required.
  • Bachelor's Degree in healthcare, behavioral healthcare, or a related field, with two years of relevant experience in a Behavioral Health system preferred.
  • Master's Degree in healthcare, behavioral healthcare, or a related field, with one year of relevant experience preferred.
  • Experience supervising personnel is preferred.

Licensure:
Current California Driver's License and Automobile Insurance.

Travel Requirements:
Occasional travel within Los Angeles County and adjacent areas may be necessary for outreach, project coordination, training, etc.
