We seek a candidate with robust leadership capabilities to guide the field operations of our street medicine and care management teams. The ideal Field Team Lead/Community Health Worker (CHW) will resonate with our mission of delivering equitable healthcare to underserved populations, meeting individuals where they reside. This Field Team Lead/CHW will act as a champion for the health needs of community members, assisting residents in effectively engaging with healthcare providers and social service organizations. Additionally, this role is integral to the site leadership team, contributing to logistics and program assessment. The position involves acting as a liaison and advocate to implement initiatives that promote, sustain, and enhance both individual and community health. Responsibilities may include delivering preventive health services such as screenings for blood pressure, glaucoma, and hearing. Utilizing community health needs assessments will be crucial in identifying areas for expanding clinical services and support.

Staff selected for this initiative will be part of a significant and sustainable street medicine program focused on assisting unhoused populations in Riverside. The most vulnerable individuals experiencing homelessness often face systemic failures, leading to mistrust of institutions and care providers. This mistrust can hinder access to necessary care and resources that could enhance their quality of life.

The street medicine team is dedicated to fostering trusting relationships with those in need of medical services, working to eliminate logistical barriers that obstruct access to healthcare, and providing care directly where unhoused individuals reside.

CHWs are frontline public health professionals who are trusted members of the communities they serve. This relationship enables CHWs to bridge health and social services, facilitating access to care and enhancing the quality and cultural relevance of service delivery. CHWs also empower individuals and communities by increasing health literacy and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.

CHWs with lived or professional experiences that resonate with the populations they serve are particularly valuable. This may encompass experiences related to incarceration, military service, pregnancy, disability, homelessness, mental health challenges, substance use, or being a survivor of domestic violence. Shared race, ethnicity, sexual orientation, gender identity, language, or cultural background can also enhance the connection between the CHW and the community served.

Key Responsibilities

  • Assist in daily site and program operations, including mapping team efforts.
  • Coordinate logistics and operations among teams to ensure the achievement of leadership objectives.
  • Oversee the team's daily goals and activities.
  • Supervise CHW team members and facilitate communication between CHWs, clinicians, and operational support.
  • Review necessary supplies for patient outreach encounters.
  • Evaluate program performance using key performance indicators and identify opportunities for improvement.
  • Conduct outreach within specific health programs and defined patient populations, engaging local businesses and organizations.
  • Perform telephonic and face-to-face outreach to identify social determinants affecting patients' health.
  • Utilize coaching and motivational interviewing techniques to support patients in reaching their health goals.
  • Employ technology and digital resources to monitor ongoing care activities.
  • Identify barriers to achieving clinical or social outcomes and collaborate with the care team to adjust care plans as needed.
  • Document all participant interactions and maintain comprehensive electronic files.
  • Assist participants in accessing health-related services and community resources.
  • Foster positive relationships with participants and collaborate effectively within a team.
  • Provide health coaching, patient navigation, and health education for a diverse patient population.
  • Ensure seamless coordination of services for members.
  • Oversee Enhanced Care Management (ECM) services and care plan implementation.
  • Connect members to necessary social services and supports.
  • Advocate for members with healthcare professionals.
  • Utilize trauma-informed care and harm-reduction approaches.
  • Coordinate with hospital staff on discharge planning.
  • Monitor treatment adherence and provide health promotion training.
  • Manage reporting requests from local, state, and federal entities.
  • Proficiently use Microsoft Office and Google Business Suite programs.
  • Collaborate with subject matter experts to articulate complex service offerings.
  • Assist in knowledge management and retention of proposal content.
  • Supervise and develop non-paid staff based on organizational needs.
  • Conduct regular team meetings to provide guidance and leadership.

Essential Skills and Qualifications:

As these roles are foundational to building the program, we are particularly interested in candidates with experience in developing outreach initiatives and engaging communities in meaningful healthcare programs.

Minimum Qualifications:

  • CHW Certificate
  • Violence Prevention Professional Certificate
  • Work Experience Pathway

Education and Experience:

  • High School diploma or GED; Associate's degree in healthcare, social work, or a related field preferred.
  • Minimum of 5 years of relevant professional or lived experience.
  • Ability to work independently and collaboratively in diverse settings.
  • Preference for candidates with Community Support Worker (CSW) or Certified Peer Support Worker (CPSW) credentials.

Preferred Skills:

  • Military experience.
  • Strong interpersonal skills and ability to follow instructions.
  • Knowledge of local and regional community resources.
  • Understanding of public health programs.
  • Proficient in industry-standard assessment techniques.
  • Ability to provide positive customer service with a patient-centered approach.
  • Comprehensive knowledge of the local community through lived experience or the ability to articulate such experiences.
  • Bilingual Spanish speakers preferred, with cultural competence in LatinX communities.
