ROLE SUMMARY

The Enhanced Care Management (ECM) Program Administrator is a pivotal role focused on the maintenance, organization, and supervision of both front-end and back-end programmatic operations. This encompasses aspects of quality assurance, compliance monitoring, and team collaboration.
This position functions within a dynamic environment where multiple responsibilities are managed simultaneously. A high degree of organizational, technological, and communication skills is essential. The individual in this role will play a crucial part in relaying information to management and will be a key resource for insights into program activities. The Program Administrator will serve as a bridge between the Street Medicine team and the Cal-AIM initiatives, ensuring that individuals from diverse cultural backgrounds receive appropriate resources and support. This role is funded through a grant for a specified duration.

KEY RESPONSIBILITIES

  • Execute clerical duties for the Enhanced Care Management and Street Medicine teams.
  • Ensure clear and consistent communication with staff regarding program meetings, community resources, and reporting requirements.
  • Collaborate effectively with other team members as directed by leadership.
  • Evaluate and process Cal Aim referrals for ECM and Community Supports-Housing Navigation & Tenancy Services.
  • Leverage Microsoft Excel for necessary data tracking and management.
  • Gather data for Cal-Aim programs, program utilization, and operational analytics.
  • Prepare reports for leadership and payers as needed.
  • Contribute to the development of policies and procedures.
  • Oversee a caseload of Enhanced Care Management clients.

Position Requirements

EDUCATIONAL BACKGROUND

  • A Bachelor's degree in Social Work, Public Health, Human Services, or a related field is required.

EXPERIENCE AND QUALIFICATIONS

  • A minimum of one (1) year of experience in a multicultural community-based environment, with foundational computer skills (Microsoft Office-Outlook, Excel, Word, Teams, Email).
  • Preferred comprehensive knowledge of Cal-Aim Programs.
  • Strong communication skills and a commitment to providing a positive experience for patients and their families.

ADDITIONAL SKILLS

  • Fluency in written and verbal English is required; bilingual proficiency in Spanish is preferred.
