ROLE SUMMARY

The Enhanced Care Management (ECM) Program Administrator is a pivotal role focused on the maintenance, administration, and supervision of both front-end and back-end program processes. This includes aspects of quality control, compliance monitoring, and team collaboration.
This position functions in a dynamic environment where multiple responsibilities are managed simultaneously. A high degree of organizational, technological, and communication skills is essential. The individual in this role will play a key part in conveying information to management and will be a trusted source for insights regarding program activities. The Program Administrator will serve as a bridge between the Street Medicine team and the Cal-AIM initiatives, ensuring that individuals from diverse cultural backgrounds receive appropriate resources and support. This role is funded through a grant for a duration of 18 months.

KEY RESPONSIBILITIES

  • Perform administrative tasks for the Enhanced Care Management and Street Medicine teams.
  • Ensure clear and consistent communication with staff regarding program meetings, community resources, and reporting requirements.
  • Collaborate effectively with other team members as directed by leadership.
  • Evaluate and process Cal Aim referrals for ECM and Community Supports-Housing Navigation & Tenancy Services.
  • Utilize Microsoft Excel for essential data tracking and reporting.
  • Gather data for Cal-Aim programs, program usage, and operational analytics.
  • Prepare reports for leadership and stakeholders as needed.
  • Develop and refine policies and procedures.
  • Manage a caseload of Enhanced Care Management clients.

Position Requirements

A. Educational Background

  • Bachelor's degree in Social Work, Public Health, Human Services, or a related field is required.

B. Experience and Qualifications

  • Minimum of one (1) year of experience in a multicultural community-based environment, with basic proficiency in computer applications (Microsoft Office-Outlook, Excel, Word, Teams, Email).
  • Preferred comprehensive knowledge of Cal-Aim Programs.
  • Strong communication skills and a commitment to providing a positive experience for patients and their families.

C. Skills and Knowledge

  • Fluency in English, both written and verbal, is required; bilingual in Spanish is preferred.
