San Bernardino County is seeking an Enhanced Care Management Lead Supervisor to oversee the development, execution, and assessment of the Community Health Workers (CHWs) involved in the Enhanced Care Management Program. This role entails acting as a primary leader for CHWs, ensuring adherence to their designated responsibilities, and participating in the recruitment process for future CHWs.
Key responsibilities include:
- Providing training to CHWs on engagement with diverse populations, including children, youth, and individuals involved with the justice system.
- Collaborating with healthcare professionals to formulate comprehensive patient care plans.
- Recommending modifications to care plans as necessary and guiding CHW navigators in managing referrals, appointments, and insurance documentation.
- Establishing and enforcing policies and procedures for the program, ensuring compliance with all relevant regulatory standards.
- Participating in the creation of assessment tools and care management guidelines.
- Monitoring legislative changes that impact the program and updating policies accordingly.
- Engaging in long-term strategic planning for the program.
- Analyzing outcome data to identify care gaps and implement improvements.
- Conducting initial assessments and screenings to evaluate patient health status and track treatment outcomes.
- Coordinating quality assurance and performance enhancement initiatives.
- Collaborating with Managed Care Plans to address program challenges and enhance service delivery.
The Enhanced Care Management (ECM) initiative is a vital aspect of CalAIM, representing a holistic, interdisciplinary approach to managing the comprehensive needs of high-cost, high-need managed care members through coordinated community-based services.

Conditions of Employment:
This is a contract position that does not confer regular County employee status. Candidates must successfully complete a background investigation, including drug testing and verification of employment history.
Work Schedule: Approximately 40 hours per week.
Travel: Travel throughout the County may be necessary, requiring reliable transportation and a valid California Class C driver license.
Qualifications:
Applicants must meet one of the following:
- Option 1: Certification as a Certified Community Health Worker with two years of post-certification experience in a relevant setting, plus one year as a lead worker or trainer.
- Option 2: A current healthcare professional license (e.g., LCSW, LMFT, LPCC, RN, LVN) with one year of post-licensure experience in a relevant program and one year as a lead worker.
Individuals with experience in population health management or enhanced care management are strongly encouraged to apply.
