About the Role:

Sycamores LLC is seeking a highly skilled and experienced Care Management Supervisor to join our team in Oakland. As a Care Management Supervisor, you will play a critical role in managing day-to-day operations and supervising Lead Care Managers (LCMs) with the CalAIM program, Enhanced Care Management (ECM).

Key Responsibilities:

  • Supervise and manage LCMs to ensure effective implementation of program practices consistent with organizational policies and treatment philosophy.
  • Work closely with staff to develop and implement strategies to improve care management services.
  • Provide guidance and support to LCMs to ensure they have the necessary skills and knowledge to perform their duties effectively.
  • Collaborate with other departments to ensure seamless delivery of care management services.
  • Develop and maintain relationships with community partners and stakeholders to ensure effective care coordination.

Requirements:

  • Bachelor's degree in a related field plus two years of direct care experience.
  • Minimum of 2 years prior supervisory experience in a related field; experience leading projects (team lead) can also be included.
  • Supervised experience in working with populations of focus: child, youth, and adults experiencing homelessness, at risk or frequent utilization of hospitals and emergency rooms, serious mental health and/or substance use, children involved in child welfare, and/or birth equity (pregnant and postpartum individuals).
  • Knowledge and experience working in the framework of a Whole Person Care approach with knowledge of the integration of physical health, mental health, and substance use.
  • Maintains all required licenses and certifications.
  • Valid California Driver License and a driving record acceptable to the Agency's insurance carrier.
