About Chinatown Service Center (CSC)

Established in 1971, CSC is dedicated to delivering multilingual health and social services, employment training, and counseling throughout Greater Los Angeles. We are expanding our team to further our mission of compassion and empowerment.

Key Responsibilities:

  • Work collaboratively with clients, their support networks, and healthcare professionals to develop and modify Health Action Plans (HAPs).
  • Ensure the objectives of HAPs are achieved while providing comprehensive care coordination.
  • Serve as a bridge between clients, healthcare teams, and external resources.
  • Deliver ECM services, connect clients to social services, and advocate on their behalf.
  • Engage clients in their care and provide training in health promotion and self-management.
  • Monitor adherence to treatment plans and assist with discharge planning.
  • Perform additional duties as required.

Essential Qualifications:

  • Bachelor's degree in a relevant field or equivalent professional experience.
  • Excellent interpersonal and analytical skills.
  • Detail-oriented with strong communication capabilities.
  • Proficient in computer and internet usage.
  • Able to work independently and manage multiple tasks effectively.
  • Bilingual in English and Chinese is preferred.
  • Experience with government program eligibility and assessments.
  • Valid California driver's license and willingness to travel as needed.
