Job Summary

We are seeking a highly skilled and experienced Regional Program Director to lead our Community Care initiatives. As a key member of our team, you will be responsible for developing and implementing strategies to drive growth and improve home health and hospice services.

Key Responsibilities
  • Represent the region in professional contacts with senior living associations, senior living facilities, assisted living facilities, independent living facilities, and personal care homes to promote available Medicare services.
  • Collaborate with leadership to strategize for growth and assist with implementation and execution of community care programs.
  • Monitor execution of the community care program and service provision through ongoing quality assurance visits with referral sources and working with operational and clinical team members.
  • Meet and exceed annual referral and admission goals as set by senior management.
Requirements
  • Graduate of an approved school of nursing or therapy (RN, LN, PT, OT, SLP) with a valid license in the state where they currently practice.
  • Demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing the operations of a home health or hospice.
  • Excellent oral and written communication skills, with strong presentation skills.
  • Ability to interact positively with a diverse population and build relationships.
  • Self-motivated and able to work independently, prioritizing multiple tasks and deadlines simultaneously.
  • Ability to maintain and adhere to multiple budgets and meet sales goals.
  • Strong understanding of customer and market dynamics, and transitional care.
