Responsibilities

The Community Care Regional Program Director is an RN, LN, PT, OT, or SLP who represents the region in professional engagements with senior living associations, senior living facilities, assisted living facilities, independent living facilities, and personal care homes to inform them of available Medicare services. Representation will also include collaborating with physicians and mid-level practitioners, private duty home care services, executive-level opportunities, and similar health organizations.

This position will work alongside leadership to strategize for growth and assist with the implementation and execution of community care programs that enhance home health and hospice services.

The Community Care Regional Program Director will oversee the execution of the community care program and service delivery through ongoing quality assurance visits with referral sources and collaboration with operational and clinical team members. This role will be accountable for meeting and exceeding annual referral and admission goals established by senior management.

Qualifications

  • Must be a graduate of an accredited nursing or therapy program. RN, LN, PT, OT, SLP. Must hold a valid license in the state of practice.
  • Must possess demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing home health or hospice operations.
  • Must have excellent oral and written communication skills.
  • Must exhibit strong presentation abilities.
  • Must be capable of positively interacting with a diverse population and successfully building relationships.
  • Must be organized and able to execute programs effectively.
  • Must be self-motivated and able to work independently.
  • Must be able to prioritize multiple tasks and deadlines simultaneously, with minimal supervision.
  • Must maintain and adhere to multiple budgets.
  • Must meet sales goals.
  • Must demonstrate motivation for sales, territory management, performance management, negotiation, sales planning, and profitability management.
  • Must have a strong understanding of customer and market dynamics, as well as transitional care.
  • Must be able to utilize reports and trends to support efforts and to understand clinical status and progress.
