The designated territory includes various locations within the Metro-Atlanta area, such as Columbus, Fayetteville, Thomaston, Macon, Warner Robins, and Milledgeville.

Key Responsibilities

The Community Care Regional Program Director is responsible for representing the region in professional engagements with senior living associations, facilities, and personal care homes to inform them about available Medicare services. This role includes collaborating with physicians, mid-level practitioners, and various health institutions.

This position will work closely with leadership to devise strategies for growth and facilitate the implementation of community care programs aimed at enhancing home health and hospice services.

The Director will oversee the execution of community care initiatives and service delivery through ongoing quality assurance assessments with referral sources, collaborating with operational and clinical teams. This role is accountable for achieving and surpassing annual referral and admission targets established by senior management.

Qualifications

  • Must be a graduate of an accredited nursing or therapy program. RN, LN, PT, OT, SLP credentials are required, along with a valid state license.
  • Demonstrated knowledge of federal, state, and local regulations governing home health or hospice operations.
  • Excellent verbal and written communication skills are essential.
  • Strong presentation capabilities are required.
  • Ability to engage positively with diverse populations and build effective relationships.
  • Proficiency in organizing and executing programs.
  • Self-motivated with the ability to work independently.
  • Capability to manage multiple tasks and deadlines with minimal supervision.
  • Experience in budget management.
  • Sales goal achievement is necessary.
  • Motivation for sales, territory management, performance oversight, negotiation, and profitability management.
  • Strong understanding of customer and market dynamics, particularly in transitional care.
  • Ability to analyze reports and trends to support clinical status and progress.
