Responsibilities

The Community Care Regional Program Director is an RN, LN, PT, OT, or SLP who represents the region in professional interactions with senior living associations, senior living facilities, assisted living facilities, independent living facilities, and personal care homes to inform them of available Medicare services. This role also involves collaborating with physicians and mid-level practitioners, private duty home care services, executive-level opportunities, and similar health organizations.

This position will work alongside leadership to strategize for growth and assist in the implementation and execution of community care programs that enhance home health and hospice services.

The Community Care Regional Program Director will oversee the execution of the community care program and service delivery through ongoing quality assurance visits with referral sources and collaboration with operational and clinical team members. This role is responsible for meeting and exceeding annual referral and admission targets set by senior management.

Qualifications

  • Must be a graduate of an accredited nursing or therapy program. RN, LN, PT, OT, SLP. Must hold a valid license in the state of practice.
  • Must possess demonstrated experience and knowledge of federal, state, and local laws and regulatory guidelines governing home health or hospice operations.
  • Must have excellent verbal and written communication skills.
  • Must exhibit strong presentation abilities.
  • Must be capable of positively interacting with a diverse population and successfully building relationships.
  • Must be able to organize and execute programs effectively.
  • Must be self-motivated and able to work independently.
  • Must be able to prioritize multiple tasks and deadlines simultaneously, with minimal supervision.
  • Must maintain and adhere to multiple budgets.
  • Must achieve sales goals.
  • Must have a strong motivation for sales, territory management, performance management, negotiation, sales planning, and profitability management.
  • Must demonstrate a solid understanding of customer and market dynamics, as well as transitional care.
  • Must be able to utilize reports and trends to support efforts and understand clinical status and progress.
