The role of the Community Care Regional Program Director encompasses the Metro-Atlanta area and surrounding regions. This position requires a dedicated professional to operate from the nearest agency office to their residence within the designated territory.

Traveling up to 80% of the week to various agency locations and accounts is an essential part of this role.

Key Responsibilities

The Community Care Regional Program Director, who must be an RN, LN, PT, OT, or SLP, will engage with senior living associations and facilities to inform them about available Medicare services. This role also involves liaising with physicians, mid-level practitioners, and various health institutions to enhance service delivery.

Collaboration with leadership is crucial for strategizing growth and executing community care programs aimed at improving home health and hospice services.

The Director will oversee the implementation of community care programs and ensure quality service delivery through regular quality assurance visits and collaboration with operational and clinical teams. Achieving and surpassing annual referral and admission targets set by senior management is a key responsibility.

Qualifications

  • Graduation from an accredited nursing or therapy program. Must hold a valid license in the state of practice.
  • Proven experience and knowledge of federal, state, and local regulations governing home health or hospice operations.
  • Exceptional oral and written communication skills.
  • Strong presentation capabilities.
  • Able to foster positive interactions with diverse populations and build effective relationships.
  • Skilled in organizing and executing programs.
  • Self-motivated with the ability to work independently.
  • Proficient in managing multiple tasks and deadlines with minimal supervision.
  • Ability to adhere to budgetary constraints.
  • Experience in meeting sales objectives.
  • Motivated in sales, territory management, performance management, negotiation, and profitability management.
  • Strong understanding of customer and market dynamics, particularly in transitional care.
  • Adept at utilizing reports and trends to support clinical status and progress.
