Overview: Under the guidance of the Program Director, the Community Support Coordinator will primarily focus on delivering supported living services to adults with developmental disabilities who choose to live independently within their community.

Key Responsibilities:

  • Conduct community outreach and provide assistance to individuals served.
  • Collaborate with the consumer's circle of support to help achieve the goals outlined in their Individual Program Plan (IPP).
  • Offer supervision, assistance, and support in specified areas of need as identified in the IPP.

The Community Support Coordinator is expected to embody the core values of People Services, Inc., which include Respect, Dignity, and Empowerment in all interactions with consumers, team members, agency partners, and the broader community.

Qualifications:

  • High School Graduate or equivalent.
  • Preferred experience in working with individuals with developmental disabilities or related fields.
  • Fingerprint Clearance and negative results from drug testing.
  • Valid California Driver's License with a clean driving record.
  • CPR and First Aid Certification, or the ability to obtain certification.
  • Ability to obtain Pro Act Certification.
  • Reliable, registered, and insured vehicle.
  • Strong communication skills, including the ability to read and interpret documents and follow verbal instructions.
  • Flexibility to accommodate the needs of consumers.

Essential Duties:

  • Maintain effective communication with the Program Director.
  • Prepare and monitor IPP for assigned clients, including training and data collection.
  • Assist consumers in identifying personal goals, which may include money management, social security assistance, and medical appointments.
  • Support consumers with housing challenges, including finding affordable housing and advocating with landlords.
  • Educate consumers about their rights and responsibilities.
  • Address challenging behaviors with respect and proactivity.
  • Build respectful relationships based on trust while maintaining professional boundaries.
  • Ensure confidentiality of all consumer information and facility operations.
  • Complete operational paperwork and reports in a timely manner.
  • Participate in interdisciplinary team meetings and staff meetings as required.
