POSITION SUMMARY
Under the supervision of the Housing Based Support Services Manager, the ICMS Case Manager is tasked with delivering integrated Case Management and Support Services to individuals who have previously experienced homelessness and have specific needs. This role involves providing on-site assistance to residents of a newly established multi-unit permanent supportive housing facility. The Case Manager will facilitate connections to various services, including but not limited to life skills training, financial management, educational opportunities, health services access, counseling, job training, and community engagement activities.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Conduct initial housing intake and needs assessments for each resident.
- Assist residents in identifying personal objectives and formulating plans to achieve these goals through the creation of Individual Service Plans (ISP).
- Address any existing challenges or barriers for clients to ensure timely completion of all housing-related processes.
- Maintain a minimum of two in-person meetings with each resident monthly.
- Facilitate quarterly reviews with residents to evaluate progress and achievements as outlined in their Individualized Stability Plans.
- Serve as a liaison between the Property Manager, property management company, residents, and primary mental health providers to ensure effective communication regarding any concerns that may impact residency.
- Provide clients with information and guidance on Fair Housing, tenant rights and responsibilities, and adherence to rental agreements and property regulations.
- Coordinate access to services with various public social service agencies and community organizations, aiding clients in achieving and maintaining independence through employment and educational activities, financial counseling, and legal assistance.
- Support clients in securing rental assistance and eviction prevention services.
- Address resident complaints, mediate disputes, and resolve issues with property management that may affect tenancy.
- Enforce housing regulations in collaboration with property management.
- Collaborate with the Resident Services Coordinator to develop workshops on essential topics such as housekeeping, financial management, parenting, and daily living skills necessary for sustaining housing and achieving independence.
- Document client progress through data entry into the DHS CHAMP database, compile reports, and evaluate program effectiveness.
- Organize and manage client files efficiently.
- Monitor the status of tenants who have transitioned into housing and prepare those ready to move to mainstream non-supportive housing.
- Adhere to documentation and reporting requirements as stipulated in the service contract.
- Compile and submit monthly reports tracking residents' status and the supportive services provided.
- Uphold the mission and strategic objectives of the Coalition for Responsible Community Development.
- 1-2 years of relevant experience and a Bachelor's Degree.
- Experience working with youth facing homelessness and housing instability, as well as familiarity with foster care, probation, and mental health systems.
- Dependable, adaptable team player capable of working independently, demonstrating a strong work ethic, and maintaining personal boundaries.
- Possession of reliable transportation, a valid driver's license, and car insurance as mandated by law.
- Proficiency in Microsoft Office Suite is required.
$48,000 - $55,000 annually
