POSITION SUMMARY

The Coalition for Responsible Community Development is seeking a dedicated individual to fill the role of ICMS Case Manager. This position is pivotal in delivering integrated Case Management and Support Services tailored for individuals transitioning from homelessness and facing unique challenges. The ICMS Case Manager will provide essential services directly at our state-of-the-art multi-unit permanent supportive housing facility. This role involves connecting clients with vital resources such as life skills training, financial literacy programs, educational opportunities, health services, counseling, job training, and community engagement activities.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Conduct initial housing intake assessments and identify individual needs for each tenant.
  • Collaborate with tenants to establish personal objectives and create Individual Service Plans (ISP) to achieve these goals.
  • Address any existing challenges or barriers that may affect clients' housing processes to ensure timely completion.
  • Engage in at least two in-person meetings with each tenant monthly to monitor progress.
  • Facilitate quarterly reviews with tenants to evaluate their achievements as outlined in their Individualized Stability Plans.
  • Serve as a communication bridge between the Property Manager, property management team, tenants, and primary mental health providers to address any concerns that may impact residency.
  • Provide tenants with guidance on Fair Housing regulations, tenant rights, and adherence to rental agreements and property policies.
  • Coordinate with various public social service agencies and community organizations to assist clients in achieving self-sufficiency through employment, education, financial management, and legal support.
  • Support clients in accessing rental assistance and eviction prevention services.
  • Resolve tenant disputes and grievances while mediating issues with property management that could affect tenancy.
  • Collaborate with property management to enforce housing regulations.
  • Work alongside the Resident Services Coordinator to create workshops on essential topics such as financial management, parenting, and daily living skills necessary for maintaining housing stability.
  • Document client progress through data entry into the DHS CHAMP database, compile reports, and assess program effectiveness.
  • Maintain organized client files and records.
  • Monitor the status of tenants who have transitioned into housing and prepare those ready for mainstream housing options.
  • Adhere to documentation and reporting requirements as stipulated in service contracts.
  • Prepare and submit monthly reports tracking tenants' residency status and the supportive services provided.
  • Uphold the mission and strategic objectives of the Coalition for Responsible Community Development.
Requirements
  • 1-2 years of relevant experience and a Bachelor's Degree.
  • Experience working with youth facing homelessness and housing instability, or in foster care, probation, or mental health settings.
  • Dependable, adaptable team player capable of working independently with a strong work ethic and the ability to maintain professional boundaries.
  • Possession of reliable transportation, including a valid driver's license and appropriate car insurance.
  • Proficiency in Microsoft Office Suite is required.
SALARY

$48,000 - $55,000 annually
