Job Overview

The Community Engagement Coordinator will play a vital role in enhancing awareness and support services aimed at educating and involving community members regarding the health care services available at Universal Community Health Center. This position is essential for ensuring continuity of care and providing crucial resources for community retention and growth.

Key Responsibilities:

  • Facilitate outreach efforts, manage appointment-related communications, and engage directly with patients through various channels.
  • Conduct regular evaluations with the Quality Director to assess outreach effectiveness and outcomes.
  • Organize and lead community activities to educate and gather feedback from residents and key stakeholders.
  • Act as a representative for Universal Community Health Center in marketing initiatives, promoting all services, including mobile clinic outreach.
  • Deliver presentations to patients and stakeholders on specific health programs and topics aimed at enhancing patient care and quality.
  • Create and distribute educational outreach materials and brochures.
  • Assist the Quality Director in formulating best practices for outreach activities.
  • Educate patients about additional support services offered by Universal Community Health Center to address social determinants of health.
  • Collaborate with enrollment coordinators to facilitate health program enrollments and patient portal access.
  • Generate periodic data reports to ensure accurate and comprehensive data collection, including communication with patients and the community.
  • Understand program objectives, monitor progress towards these objectives, and regularly communicate updates to program leadership.
  • Perform other tasks as assigned by the Quality Director.

Qualifications:

  • High School diploma or GED is required.
  • A genuine enthusiasm for community service and problem-solving.
  • A passion for engaging with people and confidence in public speaking.
  • Preferred experience in health education, outreach, and HEDIS measures.
  • Experience in outreach within diverse cultural and economic communities.
  • Proficiency in utilizing various strategies for community engagement.
  • Bilingual proficiency in English and Spanish is mandatory.
  • Flexible working hours; required to operate in both clinic and field settings.
  • Experience in community engagement and event facilitation.
  • Proficient in using Google G-Suite applications.
  • Possession of a valid California Driver's License and Insurance Certificate, with access to a reliable vehicle.
  • Strong commitment to attendance and punctuality.
  • Dedication to the mission of the organization.
  • Experience with non-profit organizations is preferred.
