Job Overview

POSITION SUMMARY

The Housing Support Services Coordinator will oversee the provision of comprehensive case management and supportive services for individuals transitioning from homelessness, particularly those with unique needs. This role involves delivering assistance directly to residents at our newly established multi-unit permanent supportive housing facility. The Coordinator will also facilitate connections to essential services such as life skills training, financial management, educational support, healthcare access, counseling, job training, and community engagement activities.

KEY RESPONSIBILITIES

  • Conduct initial housing assessments and needs evaluations for each resident.
  • Assist residents in setting personal objectives and creating actionable plans through the Individual Service Plan (ISP).
  • Address any existing challenges or barriers to ensure timely completion of housing-related processes.
  • Engage in at least two in-person meetings with each resident monthly.
  • Facilitate quarterly reviews to evaluate residents' progress based on their Individualized Stability Plans.
  • Act as a communication bridge between the Property Manager, management company, residents, and primary mental health providers to address any concerns that may affect residency.
  • Provide guidance on Fair Housing laws, tenant rights, and adherence to rental agreements and property regulations.
  • Coordinate access to services with various public agencies and community organizations to promote independence through employment, education, financial counseling, and legal assistance.
  • Support residents in securing rental assistance and eviction prevention services.
  • Resolve tenant complaints, mediate disputes, and address issues with property management that could impact tenancy.
  • Enforce housing policies in collaboration with property management.
  • Collaborate with the Resident Services Coordinator to develop workshops on essential topics such as household management, financial literacy, parenting, and skills necessary for maintaining housing and achieving self-sufficiency.
  • Maintain accurate records and data entry in the DHS CHAMP database to track client progress and generate reports for program evaluation.
  • Organize and manage client documentation effectively.
  • Monitor the status of residents who have transitioned into housing and prepare those ready for independent living.
  • Adhere to documentation and reporting requirements as outlined in the service contract.
  • Compile and submit monthly reports to track residents' status and the supportive services provided.
  • Support the mission and strategic objectives of the Coalition for Responsible Community Development.

QUALIFICATIONS

  • 1-2 years of relevant experience and a Bachelor's Degree.
  • Experience working with youth facing homelessness and housing instability, or in foster care, probation, or mental health settings.
  • Dependable, adaptable team player with strong work ethics and the ability to maintain professional boundaries.
  • Possession of reliable transportation, a valid driver's license, and car insurance as required by law.
  • Proficiency in Microsoft Office Suite is essential.

SALARY AND BENEFITS

The annual salary for this position ranges from $48,000 to $55,000. The Coalition for Responsible Community Development offers competitive benefits, including:
