Position Summary:

The Community Resource Coordinator will be instrumental in supporting the objectives of our foundation by connecting justice-involved individuals with essential services and programs. This role is designed to facilitate access to resources that address the underlying issues contributing to justice involvement, thereby promoting economic stability and reducing future legal interactions.

Key Responsibilities:

The following duties are essential to the role of Community Resource Coordinator:
  1. Establish a welcoming and inclusive atmosphere that encourages ongoing engagement with individuals from diverse backgrounds involved in the justice system.
  2. Conduct comprehensive assessments to identify immediate needs and appropriate interventions.
  3. Facilitate connections to community resources by evaluating client needs and eligibility for various programs.
  4. Ensure clients are effectively linked to services, providing follow-up support to confirm successful engagement.
  5. Manage relationships with community partners to enhance referral processes and identify new collaboration opportunities.
  6. Participate in regular meetings for knowledge sharing and optimal service delivery.
  7. Stay informed about community resources, eligibility criteria, and best practices in reducing recidivism.
  8. Act as a liaison with various community agencies to streamline support for clients.
  9. Maintain accurate records of client interactions and service referrals in accordance with program standards.
  10. Assist in program evaluation and improvement initiatives.
  11. Attend training sessions to enhance skills relevant to the role.
  12. Perform additional duties as required to support the overall operations of the foundation.
Qualifications:

Education: A high school diploma is required; an Associate's Degree is preferred.

Experience: Previous experience working with clients affected by the criminal justice system or engaging with diverse populations in social services is preferred. Lived experience in navigating these systems is highly valued.

Work Environment:
