Job Overview

Position Title: Client Support Coordinator (CSC)

Compensation: $17.25 per hour, with potential increase to $18.25 per hour and 16 hours of Paid Time Off upon achieving medication certification.

Employment Type: Full-time, 30 hours per week with a Split Shift schedule.

Shift Details:

  • Sunday: 3pm - 11pm
  • Monday: 7am - 10am / 5pm - 11pm
  • Tuesday: 5pm - 11pm
  • Saturday: 3pm - 10pm

Role Summary:
The Client Support Coordinator is responsible for providing essential habilitative assistance and care to clients within various programs offered by the Agency. This includes community living arrangements, family support services, and vocational programs. The Coordinator will oversee daily care, education, and supervision of clients, along with maintaining accurate records.

Key Responsibilities:

  • Supervise clients, ensuring awareness of their status and location at all times.
  • Assist in the development and execution of each client's Individualized Plan of Service or Individual Educational Plan.
  • Guide and support clients in daily living activities such as personal hygiene, meal preparation, shopping, and community engagement.
  • Monitor client progress towards goals and report any incidents of abuse or neglect.
  • Complete necessary documentation, including incident reports and medication error reports.
  • Implement recommendations from healthcare professionals, including therapists.
  • Organize and facilitate recreational and community activities for clients.
  • Collaborate with clinical staff, families, and external agencies to achieve client goals.
  • Maintain precise and timely records of services provided.
  • Work collaboratively with other staff members and attend required meetings and training sessions.
  • Administer medications and provide basic health care and first aid.
  • Ensure cleanliness and organization of program sites and transportation.
  • Transport clients to designated activities.
  • Participate in emergency drills as required.
  • Complete and maintain medication certification.
  • Accurately manage clients' financial accounts.
  • Perform additional duties as assigned by the Program Director or Assistant.

Qualifications:

  • Completion of the Medication Certification Program is required.
  • Must complete PMT, CPR, and First Aid training.
  • Valid driver's license with a satisfactory driving record.
  • High School Diploma or equivalent is necessary.
  • Preferred: At least one year of experience working with individuals with developmental or behavioral challenges, or an equivalent combination of education and experience. Training will be provided for the right candidate.

Physical Requirements:

  • Ability to lift and/or move up to 75 pounds.
  • Ability to exert force exceeding 100 lbs. during physical restraint procedures.
  • Ability to stand, walk, sit, use hands and arms, talk, and hear.
  • Specific vision abilities required, including close vision and the ability to adjust focus.

Language and Mathematical Skills:

  • Ability to read, interpret, and write routine reports and correspondence in English.
  • Ability to perform basic mathematical calculations.

Reasoning Skills:

  • Ability to apply common sense understanding and problem-solving skills.

Work Environment:
The work environment typically has moderate noise levels. Employees may need to travel by automobile and will be exposed to varying weather conditions.
