Position Title: Client Support Coordinator (CSC)
Compensation: $17.25 per hour, with potential increase to $18.25 per hour and 16 hours of Paid Time Off upon achieving medication certification.
Employment Type: Full-time, 30 hours per week with a Split Shift schedule.
Shift Details:
- Sunday: 3pm - 11pm
- Monday: 7am - 10am / 5pm - 11pm
- Tuesday: 5pm - 11pm
- Saturday: 3pm - 10pm
Position Summary:
The Client Support Coordinator is responsible for providing essential habilitative assistance and care to clients within various Agency programs, including community and supported living arrangements, family support services, and vocational programs. This role involves direct daily care, teaching, and supervision of clients, along with meticulous record-keeping.
Key Responsibilities:
- Supervise clients, ensuring their well-being and location are monitored at all times.
- Assist in developing and executing each client's Individualized Plan of Service or Individual Educational Plan.
- Educate, supervise, and support clients in daily living activities such as hygiene, personal care, meal preparation, and community engagement.
- Track client progress towards goals and report any incidents of abuse, neglect, or mistreatment.
- Complete necessary documentation, including incident reports and medication error reports, and implement behavioral plans as required.
- Collaborate with therapists to implement recommendations and programs.
- Create and facilitate recreational and community participation activities for clients.
- Work closely with the Clinical Director, families, schools, and other agencies to achieve client goals.
- Maintain accurate and timely records of services provided.
- Cooperate with supervisory and other staff within the Agency and external agencies.
- Attend mandatory staff meetings, training sessions, and in-services.
- Administer medications and provide routine health care and first aid as needed.
- Ensure cleanliness and organization of program sites and transportation vehicles.
- Transport clients to and from designated activities and programs.
- Participate in emergency drills as required.
- Complete and maintain medication certification.
- Ensure accurate record-keeping of clients' financial accounts.
- Perform additional duties as assigned by the Program Director or Assistant.
Qualifications:
- Completion of the Medication Certification Program is required.
- Must complete PMT, CPR, and First Aid training.
- Valid driver's license with a satisfactory driving record.
- High School Diploma or equivalent.
- Preferred: Minimum of one year of experience working with individuals with developmental and/or behavioral challenges, or an equivalent combination of education and experience. Training is available for the right candidate.
Physical Requirements:
- Ability to lift and/or move up to 75 pounds.
- Capability to exert force exceeding 100 lbs. when initiating physical restraint procedures.
- Ability to stand, walk, sit, use hands and arms, talk, and hear.
- Specific vision abilities required, including close vision and the ability to adjust focus.
Language Skills:
- Proficient in reading, interpreting, and writing routine reports and correspondence in English.
Mathematical Skills:
- Ability to perform basic mathematical calculations.
Reasoning Ability:
- Ability to apply common sense understanding and problem-solving skills.
Work Environment:
The work environment typically has moderate noise levels. Employees may be required to travel by automobile and may encounter varying weather conditions.
