Job Overview

Position Summary:
The Support Care Specialist is responsible for delivering essential assistance and care to individuals in various programs offered by Family Options, Inc., including community living arrangements, family support services, and vocational programs.

The key responsibilities encompass providing direct daily support, educating, and overseeing clients, alongside maintaining accurate records.


Core Responsibilities:
  • Oversee clients directly, ensuring awareness of their status and whereabouts at all times.
  • Assist in developing and executing individual service plans or educational strategies for clients.
  • Guide, supervise, and support clients in executing daily living tasks, including personal hygiene, meal preparation, financial management, and community engagement.
  • Track client progress towards goals and report any incidents of abuse or neglect.
  • Complete necessary incident and medication error documentation.
  • Implement behavioral strategies and emergency procedures as required.
  • Execute recommendations from therapists in occupational, physical, and speech domains.
  • Design and facilitate recreational and community engagement activities.
  • Collaborate with clinical staff, families, educational institutions, and other organizations to achieve client goals.
  • Maintain precise and timely documentation of services provided and goals achieved.
  • Work in partnership with supervisory staff and other professionals within the organization and external agencies.
  • Participate in mandatory meetings, training sessions, and professional development opportunities.
  • Administer medications and provide basic health care and first aid as needed.
  • Ensure cleanliness and organization of program locations.
  • Transport clients to designated activities and programs.
  • Engage in emergency preparedness drills.
  • Complete and uphold certification in medication administration.
  • Ensure accurate financial record-keeping for client accounts.
  • Perform additional duties as assigned by the Program Director or Assistant.

Qualifications:
  • Successful completion of a Medication Certification Program.
  • Completion of PMT, CPR, and First Aid training.
  • Valid driver's license with a satisfactory driving record.

Educational Background:
  • High School Diploma or equivalent.
  • At least two years of experience working with individuals facing developmental and/or behavioral challenges, or a comparable combination of education and experience.

Language Proficiency:
  • Ability to read and interpret documents in English.
  • Ability to write routine reports and correspondence in English.

Mathematical Competence:
  • Ability to perform basic mathematical calculations.

Reasoning Skills:
  • Ability to apply common sense and problem-solving skills effectively.

Physical Requirements:
  • Ability to communicate, hear, stand, walk, sit, use hands for handling objects, and reach with hands and arms.
  • Ability to lift and/or move up to 75 pounds.
  • Ability to exert force when necessary for physical restraint procedures.

Work Environment:
  • Moderate noise levels in the workplace.
  • Exposure to varying weather conditions while traveling.
