Position Summary:

Family Options, Inc. is looking for a dedicated Client Support Coordinator to deliver essential assistance and care to individuals enrolled in various programs. The Coordinator will oversee daily care, provide instruction, and ensure the well-being of clients, including maintaining accurate records.

Key Responsibilities:

- Supervise clients closely, ensuring their safety and awareness of their whereabouts at all times.
- Assist in crafting and executing Individualized Plans of Service tailored to each client.
- Educate, guide, and support clients in their daily living tasks.
- Track client progress towards their goals while safeguarding their rights.
- Complete necessary documentation for incidents and medication errors.
- Implement behavioral strategies and therapeutic recommendations.
- Organize and facilitate recreational and community engagement activities.
- Collaborate with the Clinical Director, consultants, families, and other relevant organizations.
- Maintain precise records of services rendered and objectives achieved.
- Participate in required meetings, training sessions, and workshops.
- Administer medications and provide routine health care as needed.
- Ensure cleanliness and orderliness of program locations and equipment.
- Transport clients to scheduled activities and programs.
- Conduct emergency drills as required.

Collaboration:

- Attend team meetings, training sessions, and workshops.
- Work in partnership with supervisory staff, agency personnel, and external partners.
- Communicate effectively and respectfully with all stakeholders involved.

Qualifications:

- Completion of a Medication Certification Program is required.
- Certification in PMT, CPR, and First Aid is necessary.
- A valid driver's license with a satisfactory driving record is essential.

Education and Experience:

- High School Diploma or equivalent is required.
- At least two years of experience working with individuals with developmental or behavioral challenges, or a comparable combination of education and experience.

Physical Requirements:

- Ability to perform essential job functions, including lifting and moving up to 75 pounds.
- Specific vision abilities may be required.
- Capability to manage physical restraint procedures.

Language Proficiency:

- Ability to read, write, and interpret documents in English.
- Proficient in writing reports and correspondence in English.

Mathematical Skills:

- Competence in basic mathematical operations.

Reasoning Skills:

- Ability to apply common sense and problem-solving skills effectively.

Work Environment:

- Moderate noise levels may be encountered.
- Travel by automobile and exposure to varying weather conditions may be necessary.
