+ Established in 1997, AdventHealth Celebration Health is a 203-bed hospital designed as a Mediterranean resort-style facility, serving as a cornerstone of health in a planned community.
+ The hospital consistently provides a state-of-the-art healing environment to residents and visitors alike, delivering innovative, research-driven clinical solutions that integrate mind, body, and spirit in overcoming illness and disease.
+ The Imaging department plays a crucial role in patient care, assisting medical staff in diagnosing and treating patients through a wide range of imaging studies and advanced interventional procedures.
+ Annually, the Imaging department performs 132,000 procedures across various settings, ensuring quality care for the greater community.

Your Role:
*The Care Management Operations Supervisor, under the guidance of the Care Management Manager or Facility Director, is responsible for overseeing daily operations and supervising team members within the Care Management department.*
*This role serves as the primary contact for Care Managers, Social Workers, and patients, addressing any barriers or concerns that may arise in inpatient units. The Supervisor is dedicated to supporting positive patient outcomes, conducting employee evaluations, and enhancing customer experience.*

Key Responsibilities:

  • Supervise daily operations of discharge planning and social services in inpatient and emergency settings.
  • Directly oversee Care Management staff and manage patient outcomes.
  • Assist in implementing Corporate Care Management strategies.
  • Ensure active participation in daily Multidisciplinary Rounds.
  • Serve as a resource for medical necessity criteria and discharge criteria.
  • Facilitate care conferences for complex transitions and placements.
  • Participate in hospital meetings to review patients with extended stays or complex needs.
  • Address complaints and issues in a timely manner.
  • Encourage team engagement and maintain open communication.
  • Assist with staff performance evaluations and corrective action plans.

Qualifications Required:
Registered Nurse:
Associate's Degree in Nursing or
Social Worker:
BSW with two (2) years of direct Care Management experience. State-specific RN license required for nurses.
Preferred Qualifications:
Registered Nurse:
BSN
Social Worker:
MSW with three (3) years of direct Care Management experience. Certifications such as CCM, ACM, or RN-BC in Care Management are advantageous.
