Position: Targeted Case Manager

We support our case managers in achieving a healthy work/life balance by promoting a hybrid work schedule, allowing flexibility to work from home, the office, or within the community.

Qualifications:

Applicants must possess a valid Wisconsin driver's license, vehicle, and auto insurance, along with a Bachelor's degree (B.A. / B.S.) from an accredited institution. Additionally, candidates should have at least one year of relevant experience or training, or an equivalent combination of education and experience.

Key Responsibilities:
  • Stay informed about the evolving resources available in the community.
  • Conduct comprehensive assessments of client needs.
  • Develop and maintain individualized recovery plans (IRPs), ensuring reviews occur at least biannually.
  • Continuously monitor the effectiveness of individual recovery plans.
  • Provide strengths-based support to clients.
  • Build and maintain effective relationships with community partners and stakeholders.
  • Offer crisis case management as necessary.
  • Maintain an empathetic, co-occurring competent, and trauma-informed approach.
  • Manage cases efficiently, utilizing authorized units effectively.
  • Occasionally transport clients to appointments and other necessary locations.
  • Collaborate with hospitals to facilitate client transitions to less restrictive environments.
  • Monitor medication efficacy and safety.
  • Ensure clients have access to safe and affordable housing.
  • Assist in the integration of clients into the community.
  • Complete time-sensitive documentation promptly.
  • Support clients in pursuing employment and educational opportunities.
  • Ensure all relevant medical providers are engaged in client care.
  • Maintain confidentiality of materials in compliance with HIPAA regulations.
  • Participate in required training, conferences, and meetings as mandated by contracts with the behavioral health division.
  • Assist clients with financial management and applications for social security benefits.
  • Act as a general advocate for clients within the community.
  • Support clients in relocating and grocery shopping as needed.
  • Attend client events and agency initiatives.
  • Engage in agency improvement projects.
  • Perform other duties as assigned.
