Position Summary:

The Recovery Support Coordinator (RSC) is responsible for delivering comprehensive recovery support services tailored to the unique needs of each assigned client.

Employment Type:

Key Responsibilities:
  • Manage an assigned caseload effectively.
  • Initiate contact with clients within 24 hours of referral or immediately for urgent needs.
  • Conduct thorough needs assessments for each client.
  • Support clients throughout their enrollment process.
  • Collaborate with clients to create a personalized Recovery Plan of Care (RPOC) based on their self-identified goals.
  • Formulate a Recovery Support Team (RST) with both formal and informal resources using the Wraparound approach.
  • Facilitate monthly RST meetings to review progress on the RPOC and make necessary adjustments.
  • Engage in weekly face-to-face meetings with clients to monitor their progress.
  • Connect clients with community resources to fulfill their RPOC objectives.
  • Coordinate care among all systems and providers involved with the client.
  • Advocate for clients while collaborating with various services, including Treatment Courts and healthcare providers.
  • Complete and submit initial Service Authorization Requests (SARs) for treatment and recovery support services.
  • Evaluate the ongoing need for RSC services and refer clients to appropriate community-based services when necessary.
  • Develop an extensive knowledge of community resources and identify natural supports for case management.
  • Utilize knowledge of procedures to address clients' emergency situations while being available 24/7 as scheduled.
  • Ensure timely and accurate completion of all required documentation related to the program.
  • Maintain and organize client care coordination files securely.
  • Provide transportation to clients as needed.
  • Ensure clients meet program requirements and follow up on appointments.
  • Participate in community networks and committees as assigned.
  • Adhere to all program and agency policies and procedures.
  • Engage professionally with clients, colleagues, and external partners.
  • Comply with all confidentiality regulations.
  • Perform additional duties as assigned by management.
Qualifications:

Preferred qualifications include an Associate's Degree in Criminal Justice, Social Work, Human Services, or related fields. Experience in case management or community work is desirable. Knowledge of local community resources and services is beneficial. Ability to work effectively with diverse populations and manage multiple responsibilities simultaneously is essential. Bilingual skills (Spanish and English) are a plus. A valid driver's license and insured vehicle are required.

Working Conditions:

Standard business hours with potential evening and weekend responsibilities. Availability for emergency situations and travel within the community is required. Home visits may be necessary.
