Key Responsibilities:

- Implement evidence-based methodologies in service delivery, including Motivational Interviewing (MI), Trauma-Informed Care (TIC), and Person-Centered Planning.
- Employ MI techniques to enhance participant engagement and success.
- Continuously assess treatment needs and adherence for all program participants, providing assessment, treatment planning, assertive case management, and crisis intervention as per established protocols.
- Prepare and submit necessary court documentation for participants as mandated.
- Coordinate team meetings with relevant agencies and treatment team members.
- Maintain safety protocols and assess potential risks while working in the community.
- Assist participants with daily living activities and connect them with community support services.
- Encourage the development of a natural support network for each participant.
- Create and manage participant service documentation, ensuring compliance with program standards.
- Maximize participant financial contributions and third-party payments for service costs.
- Maintain ongoing communication with treatment providers and relevant agencies, ensuring timely responses to inquiries.
- Conduct frequent home visits and provide transportation for participants as needed.
- Engage in continuous professional development to enhance MI skills.
- Participate in required training and complete necessary assessments.
- Foster positive relationships with stakeholders and funding entities.
Additional Responsibilities:
- Collect necessary medical records and information for participants in a timely manner.
- Provide crisis line coverage as part of a rotational schedule.
- Attend court hearings as required.
- Participate in meetings, training sessions, and staff development activities.
- Other job-related duties may be assigned as necessary.
Qualifications:
- Bachelor's Degree in a relevant human services field.
- Experience in case management and service coordination.
- Experience working with individuals facing mental health challenges or substance use disorders.
- Valid driver's license and access to a reliable vehicle.
- Ability to pass background checks as required.
Work Environment:
The position involves a mix of office and fieldwork across multiple counties, engaging with individuals participating in various programs. The role requires travel to various facilities and may involve working with individuals who may exhibit instability. Flexibility in work hours is necessary to meet participant needs.
Physical Requirements:
Frequent travel is required, along with the ability to assist participants with light physical tasks. Office work will involve computer use and sitting for extended periods.
