  • Continuously assess treatment needs and compliance, providing case management, crisis intervention, and coordination of multidisciplinary meetings as per program standards.
  • Prepare and submit necessary legal documents for program participants as required.
  • Organize team meetings with relevant stakeholders to ensure cohesive treatment strategies.
  • Maintain safety protocols and assess potential risks while working in the community.
  • Assist participants with daily living activities, including housing, financial management, and educational pursuits.
  • Encourage the development of a natural support network for each participant.
  • Document participant progress and maintain accurate service records within required timelines.
  • Adhere to quality standards and protocols for documentation and communication.
  • Maximize participant contributions towards service costs by exploring insurance and assistance options.
  • Maintain open communication with treatment providers and other relevant agencies.
  • Conduct regular home visits and provide transportation for participants as needed.
  • Engage in ongoing professional development to enhance MI skills.
  • Participate in training sessions and complete required assessments.
  • Foster positive relationships with stakeholders and funding sources.

Additional Responsibilities:

  • Collect necessary medical records and participant information promptly.
  • Provide crisis line coverage as part of a rotational schedule.
  • Represent the organization in court proceedings as necessary.
  • Attend relevant training sessions and meetings.
  • Participate in the orientation of new staff members.
  • Fulfill other job-related duties as required.

Qualifications:

  • Bachelor's Degree in a relevant human services field.
  • Experience in case management and service coordination.
  • Background in supporting individuals with mental health or substance use challenges.
  • Valid driver's license and access to a reliable vehicle.
  • Ability to pass background checks.
