Key Responsibilities:
- Facilitate and oversee community treatment plans for individuals engaged in rehabilitation programs.
- Establish and nurture a supportive therapeutic relationship with each participant.
- Implement evidence-based methodologies, including Motivational Interviewing (MI), Trauma-Informed Care (TIC), and Person-Centered Planning.
- Utilize MI techniques to enhance participant engagement and success.
- Continuously assess treatment needs and compliance, providing case management, crisis intervention, and coordination of multidisciplinary meetings as per program standards.
- Prepare and submit necessary legal documents for program participants as required.
- Organize team meetings with relevant stakeholders to ensure cohesive treatment strategies.
- Maintain safety protocols and assess potential risks while working in the community.
- Assist participants with daily living activities, including housing, financial management, and educational pursuits.
- Encourage the development of a natural support network for each participant.
- Document participant progress and maintain accurate service records within required timelines.
- Adhere to quality standards and protocols for documentation and communication.
- Maximize participant contributions towards service costs by exploring insurance and assistance options.
- Maintain open communication with treatment providers and other relevant agencies.
- Conduct regular home visits and provide transportation for participants as needed.
- Engage in ongoing professional development to enhance MI skills.
- Participate in training sessions and complete required assessments.
- Foster positive relationships with stakeholders and funding sources.
Additional Responsibilities:

- Provide crisis line coverage as part of a rotational schedule.

Qualifications:
