- Assess ongoing treatment requirements and ensure compliance through comprehensive case management, including crisis intervention and coordination of multidisciplinary meetings as per established protocols.
- Prepare and submit necessary legal documentation for program participants as mandated.
- Coordinate team meetings with relevant stakeholders.
- Maintain safety protocols and continuously evaluate potential risks in community settings.
- Assist participants with daily living activities and connect them with community support services.
- Encourage the development of a natural support network for each participant.
- Document participant services meticulously, including case notes, assessments, and progress reports.
- Adhere to quality standards and communication protocols.
- Maximize participant contributions towards service costs by exploring insurance benefits and community resources.
- Maintain ongoing communication with treatment providers and other relevant agencies.
- Conduct frequent home visits and provide transportation for participants as needed.
- Engage in professional development to enhance MI skills.
- Participate in required training and coaching sessions.
- Foster positive relationships with stakeholders and funding entities.
Additional Responsibilities:

Qualifications:
- Bachelor's Degree in a relevant human services field.
- Experience in case management and service coordination.
- Experience working with individuals facing mental health challenges or substance use disorders.
- Valid driver's license and access to a reliable vehicle.
- Ability to pass background checks.
